Localization of metastasis within the sentinel lymph node biopsies: a predictor of additional axillary spread of breast cancer?

Abstract: The risk of additional involvement of NSLN is higher in patients with PVI and it increases progressively according the histologic localization in the lymph node, from capsule, where the afferent lymphatic channel arrives, to the opposite side of capsule promoting the extranodal extension. Size of metastasis greater than 6.0 mm presents higher risk of additional lymph node metastasis.

“…LVI as a predictor of NSLNM has been reported in several studies (Silverstein et al, 2001;Viale et al, 2005;Bolster et al, 2007;Jinno et al, 2008;Fougo et al, 2009;Alvarenga et al, 2013). LVI, overall metastasis size and PSLNs/TSLNs ratio, were three predicting factors of NSLNM which were reported by Gur et al (Gur et al, 2010).…”

“…Although breast cancer is not a lethal cancer by self, metastasis to distant organs is the main cause of breast cancer mortality and is associated with poor prognosis in breast cancer patients (2)(3)(4). Spreading of tumor to auxiliary lymph node is one of the most important factors predicting metastasis of tumor cells (5,6). Conventional therapeutic strategies includes assessment of local lymph node Breast cancer as a heterogeneous sophisticated disease includes several group with discrete clinical consequences.…”

“…So, understanding of molecular mechanisms underlying metastasis of breast cancer has a vital importance to prevention, early diagnosis and also improving of therapeutic strategies (2, 7). Furthermore, during metastatic cell colonization in destination organ some cancerous phenotypes of cancer cells reverse to make possible metastatic cells colonization (6). Recent fast growing number of evidence has supported the role of miRNAs as an important contributor to many human complex diseases including breast cancer (1,8).…”
