Bu çalışmanın amacı tüm inmelerin yaklaşık beşte birini oluşturan ve yüksek mortalite ve morbidite oranlarına sahip intraserebral hematomların (İSH) klinik-radyolojik özellikleri, cerrahi tedavi endikasyonları ve sonuçlarını geniş bir klinik seriden elde edilen tecrübeler ışığında analiz etmektir. Gereç ve Yöntem: Çalışmaya Ocak 2006 -Aralık 2017 tarihleri arasında, spontan İSH nedeniyle kliniğimizde ameliyat edilen 18 yaş üstü tüm hastalar dahil edildi. Hastaların epikriz kayıtları, radyolojik görüntüleri, ameliyat notları ve takip sonuçları retrospektif olarak incelendi. Yaş, cinsiyet, risk faktörü varlığı, başvuru anındaki nörolojik durum, hematomun lokalizasyonu ve hacmi, radyolojik özellikler, cerrahi teknik, postoperatif nörolojik sonuçlar, re-operasyon ve takip sonuçlarını içeren veriler toplandı. Bulgular: Spontan İSH nedeniyle toplam 88 hasta (30 kadın (%34,1), 58 erkek (%65,9)) opere edildi. Tanı anındaki yaş ortalaması 59,3±20,1 yıl (aralık: 18-97, ortanca: 62) olarak hesaplandı. Başlıca başvuru semptom ve bulguları bilinç kaybı, bulantı-kusma, şiddetli baş ağrısı, ajitasyon, nöbet ve kuvvet kaybı (hemipleji) idi. Hastalar semptomların ortaya çıkışını takiben 20 dakika ile 5 saat arasında değişen sürelerde acile başvurdu. 74 hastaya (%84,1) dekompresif hemikraniyektomi + hematom aspirasyonu, 14 hastaya (%15,9) kraniyotomi + hematom aspirasyonu uygulandı. Ventriküle açılmış kanaması olan 58 hastanın (%65,9) 22'sinde (%25) izlemde gelişen hidrosefali nedeniyle eksternal ventriküler drenaj (EVD) ihtiyacı oldu. Antiagregan/antikoagülan kullanımı olan 37 (%42) hastanı 5'inde (%5,7) tekrarlayan kanama veya hematom boyutunda artış gözlendi. Hastaların 47'si (%53,4) kanamadan sonraki ilk 30 gün içinde hayatını kaybetti. GKS'si 8 ve daha düşük, kan hacmi ise 70 cm3'ün üzerinde olan hastalarda ise bu dönemde ölüm oranı %88 olarak bulundu. Sonuç: Antiagregan/antikoagülan kullanım öyküsü bulunan, yaşlı, hipertansif hastalarda daha yaygın görülen İSH mortalite ve morbiditesi oldukça yüksek bir hastalıktır. İleri yaş, çoklu yandaş hastalık varlığı, büyük hematom hacmi, başvuru anındaki düşük Glasgow koma skalası kötü prognoz ile ilişkilidir. Erken ve etkili cerrahi tedavi özellikle genç, yandaş hastalığı olmayan, nörolojik durumu nisbeten iyi veya ani kötüleşen hastalarda hayat kurtarıcı olabilmektedir.

Aims: To compare Endoscopic Surgery and Robot CAS-R-2 drainage for surgical treatment of basal ganglia hypertensive intracerebral hemorrhage. Methods: Fifty consecutive basal ganglia hemorrhage patients were reviewed retrospectively. The primary and secondary outcomes were 6-months mortality and 6-months modified Rankin Scale score, which were assessed by a multivariate logistic regression mode.Results: Compared with the Robot CAS-R-2 group, the 6-month prognosis was significantly higher in the endoscopic evacuation group (OR 5.912, 95% CI 1.014-34.459). Preoperative hematoma volume (OR = 0.931, 95% CI 0.875-0.990) and IVH (OR = 0.578, 95% CI0.396-0.844) were independent predictors for 6-month prognosis. No significant difference in the mortality was found in the two groups (OR 0.501, 95% CI 0.192-1.308).Conclusion: Endoscopic hematoma removal significantly improved the 6-month prognosis compared with the drainage of hematoma under Robot CAS-R-2.

The gelatin-thrombin matrix, Floseal, is an excellent novel hemostatic agent that is used in various surgical fields. Thrombin is a serine protease, and the conversion of prothrombin to thrombin is an essential step in the coagulation cascade. However, thrombin can induce blood-brain barrier (BBB) disruption and vasogenic brain edema. This report describes two cases of gelatinthrombin matrix-related cyst formation after cerebral hematoma evacuation. An 82-year-old man with a gelatin-thrombin matrix-related cyst was treated by cyst drainage and fenestration to the lateral ventricle. Histological evaluation of the cyst wall showed a gelatin-thrombin matrix reserve, marked infiltration of inflammatory cells, and foam cell accumulation. In addition, an 85-year-old woman with a gelatin-thrombin matrix-related cyst was treated with steroids and responded well. In both cases, the post-treatment course was uneventful. Cyst shrinkage and no recurrence were observed. The gelatin-thrombin matrix can cause cyst formation with brain edema. This is the first report demonstrating the cyst wall pathology and the steroid responsivity on cyst shrinkage. The mechanism of cyst formation is thought to be thrombin-induced BBB disruption. Excess gelatin-thrombin matrix should be carefully removed from the surgical beds, particularly those having a blinded space from the neurosurgical microscope.
