Local Anesthesia in Hair Transplantation

Abstract: Effective local anesthesia can be maintained throughout hair transplantation. Care must be exercised to minimize discomfort through proper technique and to minimize toxicity through judicious use of vasoconstrictors and nerve blocks and by monitoring total dosage. Constant monitoring for toxicity is required, as is early intervention in the unlikely event that warning signs should appear.

“…Also the grafts should never be implanted in symmetrical rows and columns, there should be a random arrangement to give a natural look. [89]To achieve natural results, two other aspects that need detailed discussion are angle of implantation and direction. The angle of implanting the graft should be 45degrees or less and even further acute in the temporal triangles.…”

“…Lorazepam may increase the threshold before the rare side effects of lidocaine toxicity become evident. 1 We also administer antiemetic agents to avoid any associated nausea and vomiting. The antiemetic ondansetron 4 mg is administered intravenously over 2 to 5 minutes, immediately before the induction of anesthesia.…”

“…Vibratory stimulus to the skin will reduce the pain of initial injections. [2][3][4][5] A small 0.3 mL syringe with a 31 gauge needle is used for the initial infiltration in the donor area, allowing less pressure on the skin (Figure 1). Slow injections and waiting a few seconds between injections decrease the pain caused by tissue distension.…”

“…The patient is placed in a prone position, and local anesthesia is applied as described previously. 5,6 Usually tumescence with saline is not necessary for the P-FUE procedure. The 1.0-mm punch blade is attached to the micromotor handpiece, which is basically designed to create the recipient site, and the angle along the direction of the hairs is set.…”
