Purpose To evaluate the fatigue failure load of distinct lithium disilicate restoration designs cemented on a chairside titanium base for maxillary anterior implant‐supported restorations. Materials and Methods A left‐maxillary incisor restoration was virtually designed and sorted into 3 groups: (n = 10/group; CTD: lithium disilicate crowns cemented on custom‐milled titanium abutments; VMLD: monolithic full‐contour lithium disilicate crowns cemented on a chairside titanium‐base; VCLD: lithium disilicate crowns bonded to lithium disilicate customized anatomic structures and then cemented onto a chairside titanium base). The chairside titanium base was air‐abraded with aluminum oxide particles. Subsequently, the titanium base was steam‐cleaned and air‐dried. Then a thin coat of a silane agent was applied. The intaglio surface of the ceramic components was treated with 5% hydrofluoric acid (HF) etching gel, followed by silanization, and bonded with a resin cement. The specimens were fatigued at 20 Hz, starting with a 100 N load (5000× load pulses), followed by stepwise loading from 400 N up to 1400 N (200 N increments) at a maximum of 30,000 cycles each. The failure loads, number of cycles, and fracture analysis were recorded. The data were statistically analyzed using one‐way ANOVA, followed by pairwise comparisons (p < 0.05). Kaplan‐Meier survival plots and Weibull survival analyses were reported. Results For catastrophic fatigue failure load and the total number of cycles for failure, VMLD (1260 N, 175,231 cycles) was significantly higher than VCLD (1080 N, 139,965 cycles) and CDT (1000 N, 133,185 cycles). VMLD had a higher Weibull modulus demonstrating greater structural reliability. Conclusion VMLD had the best fatigue failure resistance when compared with the other two groups.

This study aimed to investigate the effect of grinding and multi-stimuli aging on the fatigue strength, surface topography and the phase transformation of Y-TZP ceramic. Discs were manufactured according to ISO-6872:2008 for biaxial flexure testing (diameter: 15 mm; thickness: 1.2 mm) and randomly assigned considering two factors "grinding" and "aging": C-control (as-sintered); CA-control + aging; G-ground; GA-ground + aging. Grinding was carried out with coarse diamond burs under water-cooling. Aging protocols consisted of: autoclave (134°C, 2 bars pressure, 20 hours), followed by storage for 365 days (samples were kept untouched at room temperature), and by mechanical cycling (10 6 cycles by 20 Hz under a load of 50% from the biaxial flexure monotonic tests). Flexural fatigue strengths (20,000 cycles; 6 Hz) were determined under sinusoidal cyclic loading using staircase approach. Additionally, surface topography analysis by FE-SEM and phase transformation analysis by X-ray Diffractometry were performed. Dixon and Mood methodology was used to analyze the fatigue strength data. Grinding promotes alterations of topographical pattern, while aging apparently did not alter it. Grinding triggered t-m phase transformation without impacting the fatigue strength of the Y-TZP ceramic; and aging promoted an intense t-m transformation that resulted in a toughening mechanism leading to higher fatigue strength for as-sintered condition, and a tendency of increase for ground condition (C < CA; G = GA). It concludes that grinding and aging procedures did not affect deleteriously the fatigue strength of the evaluated Y-TZP ceramic, although, it promotes surface topography alterations, except to aging, and t-m phase transformation. Objectives The aim of this study was to evaluate the effect of cyclic mechanical loading on the fracture resistance of 3D-printed zirconia crowns in comparison to milled zirconia crowns. Materials and methods Monolithic zirconia crowns (n = 30) were manufactured using subtractive milling (group M) and 3D additive printing (group P). Nine samples of each group were fractured under one-time loading while the other 6 samples were subjected to cyclic loading for 1.2 million cycles before being subjected to one-time loading until fracture. Scanning electron microscope (SEM) fractographic analysis was carried out on fractured fragments of representative samples. Results The mean for fracture resistance of group M was 1890 N without cyclic loading and 1642 N after being subjected to cyclic loading, and they were significantly higher than that of group P (1658 N and 1224 N respectively). Conclusions The fabrication technique and cyclic loading affect the fracture resistance of zirconia crowns. Although the fracture resistance values for the 3D-printed crowns were lower than those of the milled, still they are higher than the masticatory forces and thus could be considered being clinically acceptable. Clinical relevance Concerning fracture resistance, 3D-printed crowns can withstand the masticatory forces for the long term without any cracks or failure.
