Living with Encapsulating Peritoneal Sclerosis (Eps): The Patient's Perspective

Abstract: ♦ Background: Although relatively rare, encapsulating peritoneal sclerosis (EPS) is nonetheless a major concern within the renal community. Risk of developing EPS is associated with long-term peritoneal dialysis (PD). High mortality was previously reported, although surgery has since improved outcomes. Research into EPS focuses on imaging and early detection methods, genetics, biomarkers and preventive strategies. No previous studies have examined patients' experiences of EPS. ♦ Aims: The aim of the present st… Show more

“…[ 11 ] did not restricted the time of withdrawal. Further, we postulate that over the course of prolonged PD, more patients are likely to transfer to HD due to ultrafiltration failure or to avoid the development of encapsulating peritoneal sclerosis [ 12 , 13 ]. Notably, when restricted to the first 90 days of treatment, similar to our findings, the main reason for PD discontinuation during the first 90 days of treatment in the Kolesynk et al.…”

“…computed tomography [CT] scan appearance) and functional features (intermittent subacute bowel obstruction). It is important to be clinically aware of the possibility of EPS for many years after stopping PD; failure to listen to the patient and his/her symptoms may lead to a delay in diagnosis (50). Encapsulating peritoneal sclerosis presents after withdrawal from PD in the majority (70 -90% in some series) of patients (11,51) and the time from cessation of PD until the development of EPS has been reported as up to 5 years (52).…”

“…Of note, the low osmotic symptoms were often subtle but not understood. Providing written information is recommended to give the patients control and enable them to discuss their concerns with the healthcare team (50). Other strategies that have been suggested for EPS prevention include minimization of dialysate glucose administration and prescription of "biocompatible" PD fluids.…”
