Liver Transplantation Following Yttrium‐90 Radioembolization: 15‐Year Experience in 207‐Patient Cohort

Abstract: Background and Aims Radioembolization (yttrium‐90 [Y90]) is used in hepatocellular carcinoma (HCC) as a bridging as well as downstaging liver‐directed therapy to curative liver transplantation (LT). In this study, we report long‐term outcomes of LT for patients with HCC who were bridged/downstaged by Y90. Approach and Results Patients undergoing LT following Y90 between 2004 and 2018 were included, with staging by United Network for Organ Sharing (UNOS) tumor‐node‐metastasis criteria at baseline pre‐Y90 and pr… Show more

“…However, many challenges remain, such as adequate patient selection, management of post-transplant recurrence, and refinement of neoadjuvant treatment protocols. The current literature describes multiple neoadjuvant and downstaging strategies as a bridge to LT in patients with HCC, with variable results [ 27 , 28 , 29 , 30 , 31 , 32 ]. Moreover, downstaged HCC patients with tumor burdens meeting United Network for Organ Sharing (UNOS) downstaging inclusion criteria had similar post-LT outcomes compared to patients transplanted within the Milan criteria [ 33 ].…”

“…A small amount of HCC tissue remains at the surgical margin after the resection, which is one of the main causes of HCC recurrence. [ 55,56 ] The intraoperative imaging methods are needed to enable more precise and direct visualization of HCC margins and also detection of some invisible and small microscopic lesions. As a result, Andreou et al.…”

“…On the other hand, transplantation is very limited in Asia [65]. Nevertheless, resection has been frequently used to effectively treat HCC patients worldwide, and recent strategies of downstaging or bridging more patients to transplantation by locoregional therapy brought significant benefits to HCC patients [66,67]. Figure 3 also shows that the patent publications in biological therapy began to gradually rise after 2000.…”
