Background and Aims: Autoimmune hepatitis (AIH) is a rare indication (<5%) for liver transplantation (LT). The aim of this study was to describe the early outcome after LT for AIH. Methods:A multicenter retrospective nationwide study including all patients aged ≥16 transplanted for AIH in France was conducted. Occurrences of biliary and vascular complications, rejection, sepsis, retransplantation and death were collected during the first year after LT.Results: A total of 344 patients (78.8% of women, 17.0% of (sub)fulminant hepatitis and 19.2% of chronic liver diseases transplanted in the context of acute-on-chronic liver failure [ACLF]) were included, with a median age at LT of 43.6 years. Acute rejection, sepsis, biliary and vascular complications occurred in respectively 23.5%, 44.2%, 25.3% and 17.4% of patients during the first year after LT. One-year graft and patient survivals were 84.3% and 88.0% respectively. The main cause of early death was sepsis. Case report: We present a young female of fourteen years who was admitted to the hospital with short duration of Icterus, malaise, vomiting and diagnosed to be having acute hepatitis B. She went into acute liver failure as evidenced by development of hepatic encephalopathy and coagulopathy. She was managed on lines of hepatic encephalopathy along with oral antiviral treatment. She recovered successfully and was discharged after two weeks in heamodynamically stable condition. After a gap of six months, she became Hepatitis B surface (HbsAg) & hepatitis B e-antigen (HbeAg) negative and Hepatitis B Virus DNA (HBV DNA) was undetectable with normal liver function tests. She is on regular follow up for last one year and is absolutely normal. Conclusion: Acute hepatitis B can go into acute liver failure in 1% of cases, treatment for which includes liver transplantation and oral antiviral treatment which is especially helpful in cases who cannot afford liver transplantation, as was in our case.

Сеченова» Минздрава России (Сеченовский университет), Москва, Российская Федерация Болезнь Вильсона-Коновалова -редкое наследственное заболевание, вызванное недостаточностью фер-мента медь-транспортной-Р-АТФазы-В. Варианты течения заболевания колеблются в широких преде-лах от латентной формы до острой печеночной недостаточности, наблюдаемой в 5% случаев болезни Вильсона-Коновалова. В данном клиническом наблюдении представлен опыт трансплантации печени как единственного радикального метода лечения больных с фульминантной формой болезни Вильсона-Коновалова, демонстрирующего отличные послеоперационные результаты. Родственная трансплантация фрагмента печени позволяет выполнить операцию в кратчайшие сроки, что, безусловно, необходимо при острой печеночной недостаточности. Проведение сеансов обменного плазмафереза с замещением плаз-мой AB(IV) позволяет подготовить пациента к не совместимой по группе крови трансплантации.Ключевые слова: трансплантация печени от родственного донора, болезнь Вильсона-Коновалова, фульминантная печеночная недостаточность, AB0несовместимая трансплантация.
