Liver abscess complicating transcatheter arterial embolization

Abstract: Liver abscess is a rare complication after TAE for hepatocellular carcinoma. Recurrent fevers after an initial symptom free interval should arouse suspicion of an abscess. The mortality is high and a large abscess and higher age predict an unfavourable outcome. Abscess formation can lead to complete tumour resolution.

“…Infectious complications are rare after TACE because of antiseptic measures 6 and the incidence of liver abscess is described in literature as 0.26%. 10 However, the incidence of post TACE liver abscess is higher among patients with bilio-enteric anastomoses, 11 which was the case of this patient.…”

“…The microbiological cultures in the present study suggested a wide variety of causative organisms, including gram negatives such as E coli , (52.4%, 11/21) and E cloacae , (38.1%, 8/21), as well as gram positives such as E faecalis (28.6%, 6/21) and Staphylococcus epidermidis (9.5%, 2/21), which is different from the earlier studies. 6 , 18 A consensus on which antibiotic agent should be the first choice remains lacking to date. According to our bacterial spectrum, if liver abscesses following TACE are suspected, 3rd-generation cephalosporin and vancomycin should be used as the first-line antibiotics for obtaining a whole coverage of gram-positive and gram-negative organisms.…”

“…Pyogenic hepatic abscess after HAE is rare, but is associated with significant morbidity and mortality,(610) especially in patients with advanced malignancy. (1415) Moreover, abscess formation impairs recovery in patients who already suffer from terminal malignancy and may preclude future embolization procedures or other cancer-directed therapy.…”
