Background: Elevated sound levels and low language exposures of preterm infants (< 37 weeks) cared for in the neonatal unit contribute to poorer growth, cognition, language and motor outcomes. These delays can have lasting effects on childhood development and continue throughout adult life. Whilst recommendations have been established for appropriate sound exposure levels in neonatal units, very little is known about the optimal level of language exposure. Objectives: To examine the evidence regarding language exposure, both measured (observational) and prescribed (interventional), in preterm infants (< 37 weeks) cared for in neonatal units and to identify optimal exposure levels to promote neurodevelopment. Search Methods: An electronic search of English-language articles with an open date range was conducted in the PubMed, MEDLINE, Joanna Briggs, CINAHL and Cochrane databases and in Google Scholar. The criteria were limited to original research studies of preterm infants (< 37 weeks) in which language was either measured as it naturally occurred in the neonatal unit environment or through a language intervention. Results: The search produced a total of 2,367 articles, 49 of which were related to language. A full-text review of these articles identified 10 eligible studies and 6 studies from reference searches. Conclusions: Preterm infants experience low levels of language exposure and high sound exposures in neonatal units. There is a lack of conclusive evidence to recommend an optimal level of language exposure to support improved neurodevelopmental outcomes in preterm infants. Further research from large, good-quality clinical trials is required before clear direction can be provided to inform clinical practice.

Abstr Abstract actThis study investigated the contributions of music therapy to the mother-preterm infant dyad in a NICU. A single case study was carried out involving a mother and her preterm daughter (27 GW). They participated in nine sessions of the Music Therapy Intervention for the Mother and her Preterm Infant -MUSIP, with the aim of sensitizing the mother to the importance of singing. After the MUSIP and in the preand post-discharge periods, the mother was interviewed in order to provide an assessment of the intervention; furthermore, the dyad was videotaped during singing and non-singing interactions. The interviews with the mother and the written descriptions of the video footage were examined through thematic analysis. The results showed music therapy contributed: a) to the empowerment of the infant, through relaxation, stabilization of oxygen saturation, the presentation of new competences and engagement in singing; and b) to the empowerment of the mother, through relaxation, the overcoming of embarrassment and the fear of interacting with the infant, and through the strengthening of her maternal competences and her autonomy in singing. Together, the results highlighted the importance of music therapy to the mother-infant interaction, because singing contributed to more prolonged face-toface contact and more diversified expressions of affection.

RESUMOIntrodução: Os sons fazem parte de nossa vida e são capazes de produzir efeitos sobre o equilíbrio físico e emocional das pessoas. Objetivos: Avaliar os efeitos do canto materno em recém-nascidos prematuros utilizando como parâmetros a variação da frequência cardíaca, a frequência respiratória e a concentração dos bebês, bem como a percepção da mãe quanto ao comportamento do bebê durante a exposição ao canto materno. Metódos: Trata-se de uma investigação exploratória com análise quantiqualitativa. Um total de 20 bebês prematuros foi estudado durante 3 dias consecutivos. Cada criança foi exposta ao canto materno por 3 períodos de 10 minutos. A coleta foi efetuada antes do início e imediatamente após a exposição ao canto. Nesses dois momentos foram registradas a frequência cardíaca e respiratória, e a concentração de oxigênio sanguíneo do bebê. Foram analisadas e comparadas as alterações pré e pós-exposição. No terceiro dia, a mãe foi entrevistada para obtenção da percepção dela a respeito do comportamento do bebê durante a exposição ao canto. A coleta de dados foi realizada no período de março a julho de 2015 em duas maternidades conveniadas ao sistema público de saúde, em Sorocaba, São Paulo. Para a análise estatística, foram aplicados testes de Wilcoxon e Mc Nemar, e análise de variância de Friedman, apontando para não significância dos resultados. Resultados e Conclusão: Não foi observada diferença significante, porém a análise qualitativa demonstrou resultados positivos nos discursos obtidos das mães, tais como alterações comportamentais benéficas, reconhecimento da voz materna e auxílio na consolidação do vínculo mãe-bebê. Palavras-chave: musicoterapia; cuidados de enfermagem; prematuro; adaptação fisiológica; humanização da assistência. ABSTRACT Introduction: Sounds are part of our life and are able to produce effects on physical and emotional balance of people. Objective: To evaluate the effects of mother's singing on the preemie using as parameters the heart and respiratory frequency and blood oxygen concentration of preemies, and also the mother perception related to baby behavior during the exposition to her singing. Methods: It is an exploratory investigation with quantitative and qualitative analyzes. Twenty preemies babies were observed during three consecutive days. Each child was exposed to the mother singing during ten minutes for three periods. The data were collected before and immediately after the music exposition. The heart and respiratory frequency and blood oxygen concentration of the baby were registered in two moments: pre and postmusic exposition; the two measures were analyzed and compared. In the third day, the mother was interviewed to know her perception related to baby behavior during the singing exposition. The assessment of data was performed from March to July 2015 in two public hospitals in Sorocaba, São Paulo, Brazil. For the statistical analysis, Wilcoxon, Mc Nemar and variance test of Friedman were applied. Results and Conclusion: Statistics showed no significant result...
