Live counselor contact in a Facebook intervention predicts smoking cessation outcomes.

Abstract: Digital smoking cessation interventions frequently use automated delivery of content. Integrating a counselor may improve participant engagement and facilitate health behavior change. We investigated engagement with live counseling compared to automated content in a Facebook intervention and the impact of engagement on smoking cessation outcomes. “…The addition of synchronous human support through skills-based coaching appears to improve adherence to and outcomes from digital mental health interventions as well as lower dropout among adolescents [ 17 ]. For example, live counselor contact was associated with improved clinical outcomes from a smoking cessation intervention delivered via Facebook to young adults [ 21 ]. Peer-to-peer counseling from paraprofessionals in a digital environment aided the delivery of evidence-based skills to undergraduate college students [ 22 ].…”

“…A Facebook intervention has promise as a scalable intervention strategy to promote engagement in cessation treatment among AN people, especially those living in rural, remote, off-road communities, and this study points to new directions for research into how peer support through social media can promote long-term cessation in this population. Based on suggestions by our participants and supported in the literature, 49 we plan to hold Facebook live events with Native Elder speakers, Tribal cessation program counselors, Alaska’s Tobacco Quitline coaches, and or health care providers to motivate and guide cessation. That more than 70% of our participants indicated an interest in moderating a Facebook cessation group was unexpected.…”

“…While automated text messages show promise for e-cigarette cessation 21 and have an established evidence-base for cigarette smoking cessation 22 , some users find these programs impersonal and prefer being able to interact with a real person 23,24 . Automated programs can be enhanced by incorporating human-delivered counseling, and emerging evidence from text messaging and social media-based interventions suggests that this hybrid approach may be more effective and engaging than exclusively automated programs [25][26][27] . Considering the nuances of e-cigarette use, including co-use with other substances, and varying motives for use and levels of readiness to quit 28 , the addition of humandelivered counseling to automated text messages could provide more intensive, personalized support to help people who use e-cigarettes to successfully quit.…”
