Rationale: Cases of intussusception caused by mucinous carcinoma have been rarely reported, and those caused by colonic mucinous adenocarcinoma (MAC) with distant metastasis were even fewer. Patient concerns: A 60-year-old woman who complained of severe pain around the navel with nausea and vomiting for a week was admitted on November 28, 2017. There were multiple watery stools and abdominal pain was worsened over the prior week. Diagnosis: She was diagnosed by abdominal computed tomography, current medical history, and abdominal signs. Her initial diagnosis was acute abdomen, intussusceptions, and intestinal obstruction. The final diagnosis was MAC, which was based on postoperative pathology. Interventions: The patient received emergency laparotomy, followed by 5 courses of chemotherapy with oxaliplatin plus capecitabine, and then 6 courses with 5-fluorouracil + oxaliplatin + calcium leucovorin. Outcomes: The patient was in good nutritional condition, and no obvious tumor recurrence or metastasis was found until July 9, 2018. Lessons: Even though the prognosis of colonic MAC is poor, being able to receive timely surgical treatment, good nutritional status and reasonable postoperative chemotherapy are the key factors to prolonging patient's survival.

Background: Surgery is the treatment option for wide range of benign esophageal diseases. But there are very few studies on Indian population, documenting the profile and clinical outcome of esophageal disease treated by surgical intervention. Hence author undertook a study to determine the clinical outcomes following esophagectomy for benign esophageal diseases. Methods: The current study was a prospective observational study, conducted in Velammal Medical College, Madurai. Data collection was done for 6 years from January 2012 to December 2017. Patients diagnosed with benign esophageal diseases were enrolled. The surgical procedures performed and outcomes were documented. Mean and standard deviation were used to summarize numeric variables, number and percentage were used to summarize categorical variables. Results: A total of 20 cases were included, with 11 men and 9 women. The mean age was 42.6±8.3 years. Dysphagia (90%) was the most common presentation, followed by heart burn (58%), regurgitation (40%) and chest pain (25%). Boerhaave syndrome (25%) was the most common diagnosis followed by foreign body perforation in 2(10%) patients. Corrosive stricture, peptic stricture, perforation following pneumatic dilatation and end stage achalasia were present in 2(10%) patients each. Jejunostomy and pyloroplasty were the procedure done in all cases. Stomach was the conduit used in 90% of patients. The conduit placement was done in the posterior mediastinum. The duration of the hospital stay ranged from 10 to 35 days. Mortality rate was 10%. Pulmonary morbidity (40%) was the most common post-operative complication, followed by wound infection (20%). Anastomotic stricture occurred among 30% of subjects. Conclusions: There is a high probability of occurrence complications following esophagectomy for benign disease. Clinicians must be aware of incidence of various complications, to be able to minimize and manage them effectively.

Surgical repair of caustic oesophageal injuries with autologous gastrointestinal segments is often associated with dysmotility, dysphagia and donor site morbidity, and therefore alternative graft options are needed. Bilayer silk fibroin (BLSF) scaffolds were assessed for their ability to support functional restoration of damaged oesophageal tissues in a rat model of onlay oesophagoplasty. Transient exposure of isolated oesophageal segments with 40% NaOH led to corrosive oesophagitis and a 91% reduction in the luminal cross-sectional area of damaged sites. Oesophageal repair with BLSF matrices was performed in injured rats (n = 27) as well as a nondiseased cohort (n = 12) for up to 2 months after implantation. Both implant groups exhibited >80% survival rates, displayed similar degrees of weight gain, and were capable of solid food consumption following a 3-day liquid diet. End-point μ-computed tomography of repaired sites demonstrated a 4.5-fold increase in luminal cross-sectional area over baseline injury levels. Reconstructed oesophageal conduits from damaged and nondiseased animals produced comparable contractile responses to KCl and electric field stimulation while isoproterenol generated similar tissue relaxation responses. Histological and immunohistochemical evaluations of neotissues from both implant groups showed formation of a stratified, squamous epithelium with robust cytokeratin expression as well as skeletal and smooth muscle layers positive for contractile protein expression. In addition, synaptophysin positive neuronal junctions and vessels lined with CD31 positive endothelial cells were also observed at graft sites in each setting. These results provide preclinical validation for the use of BLSF scaffolds in reconstructive strategies for oesophageal repair following caustic injury.
