Liquid biopsy - emergence of a new era in personalized cancer care

Abstract: The most successful treatment for cancer involves identifying druggable, biological markers for targeted therapy. In the clinical setting, surgical removal of tumors is the only procedure for identifying such targetable molecules. Shed from tumor cells, these markers are also present in circulating blood, albeit in very negligible amounts. Liquid biopsy is a procedure performed on a blood sample to look for such circulating cancer markers cells or pieces of nucleic acid from the tumor. The procedure shows prom… Show more

“…Insufficient tissue specimens, for example because of inaccessible tumor location or late presentation with metastases at diagnosis, limit the available diagnostic testing options. 54 For example, patients with lung cancer, particularly small cell lung cancer, are typically diagnosed through cytologic material only. 55,56 However, large cell carcinomas of the lung cannot be identified with small biopsy or cytology specimens, and their diagnosis requires an invasive resection specimen.…”

“…The first and foremost step in attaining valuable information pertaining to cancer treatment is efficient isolation of CTC, cfDNA, and cfRNA shed by the tumour cells, which are very few in amount, in order to gain information and increase sensitivity and specificity. [ 6 ] Molecular analysis should be performed, followed by isolation; it includes fluorescence in situ hybridisation (FISH), microarray, sequencing, immunofluorescence, flow cytometry, and reverse-transcriptase PCR (RT-PCR). Digital PCR comprising droplet-based systems (ddPCR), microfluidic platforms for parallel PCR, NGS, BEAMing (Beads, Emulsions Amplification and Magnetics), and amplification-refractory mutation system (Scorpion-ARMS) assay are additional techniques that can also be considered.…”
