Liposuction is one of the most popular treatment modalities in aesthetic surgery with certain unique anaesthetic considerations. Liposuction is often performed as an office procedure. There are four main types of liposuction techniques based on the volume of infiltration or wetting solution injected, viz dry, wet, superwet, and tumescent technique. The tumescent technique is one of the most common liposuction techniques in which large volumes of dilute local anaesthetic (wetting solution) are injected into the fat to facilitate anaesthesia and decrease blood loss. The amount of lignocaine injected may be very large, approximately 35-55 mg/kg, raising concerns regarding local anaesthetic toxicity. Liposuction can be of two types according to the volume of solution aspirated: High volume (>4,000 ml aspirated) or low volume (<4,000 ml aspirated). While small volume liposuction may be done under local/monitored anaesthesia care, large-volume liposuction requires general anaesthesia. As a large volume of wetting solution is injected into the subcutaneous tissue, the intraoperative fluid management has to be carefully titrated along with haemodynamic monitoring and temperature control. Assessment of blood loss is difficult, as it is mixed with the aspirated fat. Since most obese patients opt for liposuction as a quick method to lose weight, all concerns related to obesity need to be addressed in a preoperative evaluation.

Fluid management in extensive liposuction

Tumescent anesthesia makes it feasible to perform liposuction in an office setting. There are often patients who desire extensive liposuction on approximately 30% of total body surface area, which means the potential of fluid overload. In this study, the charts of 83 patients undergoing extensive liposuction were retrospectively reviewed. The intra-operative fluid ratio was 1.66 for the extensive liposuction. There were no episodes of pulmonary edema, congestive heart failure exacerbation, or other major complications. The average urine output in the operating room, the recovery room, and while on the floors was 1.35, 2.3, and 1.4 mL/kg/hour respectively. Intravenous (IV) fluid administration during operation was minimized to approximately 300 to 500 mL. The total volume of IV injection was also reduced to less than 1500 mL when the patient was in the recovery room and on the hospital floor. Our fluid management strategy in extensive liposuction reflects minimal risk of volume overload. Foley catheters are not applied and patients could resume oral intake in usual, so they can discharge after 6 hours of recovery room stay in our daily practice.

Liposuction is a cosmetic surgery which is gaining popularity in present day modern world. This procedure in considered to be safe and very rarely leads to fatality. The authors report a rare case of Intra-operative death due to fat embolism during liposuction operation. The deceased had minimal prediagnosed risk factors for developing the complication while undergoing abdominal liposuction. Autopsy revealed hematoma over pericardium with luminal blockade of all coronary vessels. Hematoma and fat coagulum mixed masses were spread in between the peritoneal wall and subcutaneous tissue and. Histopathology examination of lungs and brain were done where fat embolism was present in lungs and meningeal vessels. Fat embolism syndrome (FES) is a lifethreatening condition which has been described in patients after liposuction both mechanically and biochemically. Sudden deaths in a surgical procedure such as liposuction can easily lead to allegations of medical negligence. A thorough autopsy, along with histopathological examination can reveal the actual cause of death, as evident in this case. The authors aim to increase the awareness of public and Medical profession regarding the fatal complication of aesthetic procedures which are considered generally harmless.
