Aim: The present study was performed to evaluate the longevity and efficacy of the lip repositioning surgery by assessment of the gingival display changes that occur after the surgery through clinical & digital methods.Methods: This prospective study included ten female patients who suffered from excessive gingival display due to upper lip hypermobility or short upper lip. Pre-operative measurement of the amount of gingival display were recorded both clinically using an endodontic ruler & digitally using Adobe Photoshop cs6 through a well standardized photographs. The surgery was performed under local anesthesia for all patients by removing part of the labial mucosa (labial vestibule) followed by suturing of the wound by interrupted 0-5 resorbable suture. All patients were followed up at 14 days, 4 months, 8 months and 12 months, postoperative assessment was done by measuring the changes in the gingival display using the same clinical & digital techniques.Results: Post-operative measurements at 14 days follow up, showed obvious reduction in the amount of gum exposure. However complete relapse was observed at 4 months. Conclusion:The longevity of the procedure is questionable with almost complete relapse.

BackgroundThis cross‐sectional study aimed to examine upper lip (UL) and smile characteristics and soft tissue excessive gingival display (EGD) etiologies (hypermobile upper lip [HUL], altered passive eruption [APE], and short upper lip [SUL]) in a nondental adult population and to analyze interracial (Black and White) and intergender differences.MethodsCommunity participants, non‐Hispanic Blacks (NHB) and non‐Hispanic Whites (NHW), were recruited and examined for UL vertical dimensions at rest and maximum smile and for HUL, APE, and SUL. Associations between gingival display (GD) or EGD and UL anatomical characteristics, HUL, APE, and SUL were analyzed.ResultsParticipants included 66 NHB and 65 NHW adults. Ergotrid height (greater among NHW; p = 0.019) averaged 14.0 mm. Upper lip vermilion length (ULVL), total UL length, internal lip length, total UL length during smile, and UL mobility averaged 8.6, 22.5, 23.1, 16.6, and 5.9 mm, respectively (all significantly greater in NHB; p ≤ 0.012). SUL prevalence was 4.6%, found only among NHW. Lip length change from rest to smile (LLC) averaged 26.2% (significantly greater in females; p = 0.003). HUL prevalence was 10.7% (NHB 13.1%, NHW 3.5%; p = 0.024). NHB had significantly greater GD (p ≤ 0.017). EGD and APE prevalence (6.9% for both) showed significant interracial and intergender differences (p ≤ 0.014). Multivariate logistic regression analyses indicated that LLC and HUL were the most consistently significant EGD determinants.ConclusionsUL anatomical and functional characteristics and soft tissue‐related EGD etiologies exhibit significant interracial and intergender differences, with UL mobility/hypermobility being the most consistently significant determinant of GD.

Excessive display of the gums when smiling, known as gummy smiles, can lead to aesthetic concerns and emotional discomfort for individuals. In recent times, there has been a notable rise in the popularity of gummy smile surgery as a cosmetic procedure aimed at addressing this problem. A novel technique known as lip repositioning has emerged and gained recent popularity, either on its own or in conjunction with other approaches. In specific situations, it offers a simpler, less intricate alternative to more extensive surgical procedures, resulting in a pleasing and satisfactory camouflage effect while reducing postoperative complications. This case report illustrates the effective treatment of a gummy smile in a patient with excessive gum exposure using a lip repositioning technique. The procedure involved the removal of a partial thickness strip of mucosa lining in the maxillary buccal vestibule, without including the labial frenulum, and suturing the lip lining to the gum line. Our aim was to present lip repositioning as an effective method for reducing excessive gum exposure and addressing any unevenness in a minimally invasive manner. We sought a solution that offers long-lasting results over time while minimizing the chances of the issue recurring, with a 14-month follow-up.
