Linear IgA bullous dermatosis (LABD) is a rare blistering skin disease characterized by basement membrane zone deposition of IgA. This study identifies a tissue antigen detected by patient serum and then isolates the autoantibody using epidermis and protein bands blotted on nitrocellulose as immunoabsorbents.Sera from 10 patients (9 with cutaneous disease and 1 with cicatrizing conjunctivitis) were evaluated. Indirect immunofluorescence revealed an IgA anti-basement membrane antibody in 6 of 10 sera with monkey esophagus substrate and 9 of 10 sera with human epidermal substrate. Immunoblotting was performed on epidermal and dermal extracts prepared from skin separated at the basement membrane zone with either sodium chloride or EDTA. Saline-separated skin expressed a 97-kD band in dermal extract alone that was recognized by 4 of 10 sera. EDTA-separated skin expressed the 97-kD band in both epidermal (4 of 10 sera) and dermal (6 of 10 sera) extract. Immunoabsorption of positive sera with epidermis purified an IgA antibody that reacted uniquely with the 97-kD band. In addition, IgA antibody bound to nitrocellulose was eluted from the 97-kD band and found to uniquely bind basement membrane zone.

The presentation can be similar to other bullous dermatoses, yet it has distinctive clinicopathologic and immunologic features that allow prompt recognition and treatment with complete resolution. Case Report: A 54-year-old obese Caucasian male with past medical history of atrial fibrillation on warfarin, hypertension, gastroesophageal reflux disease, benign prostatic hyperplasia, and dyslipidemia presented to the emergency department complaining of a generalized blistering rash that initially surrounded the genitalia a week after being discharged from the hospital following a mechanical fall. All medications were reviewed and skin biopsy was taken. He developed the drug-induced variant of LABD to vancomycin with mucosal involvement and compare the resemblance to other autoimmune blistering diseases such as toxic epidermal necrolysis (TEN) and Stevens-Johnson syndrome. This case demonstrates complete resolution of the disease with prompt identification of the underlying disease process based on the clinical and immunohistochemistry findings. Conclusion: Linear IgA bullous dermatosis can be difficult to diagnose as it presents similar to other bullous dermatoses. The problem of differential diagnosis coupled with clinicopathologic and immunologic features of LABD are emphasized to recognize this distinct disease.

Presentación de casos clínicos RESUMENLa dermatosis ampollar por inmunoglobulina A lineal es una rara enfermedad, generalmente autolimitada, que afecta a niños de 4,5 años (edad media), con una incidencia de 0,5-2,3 casos/millón de habitantes/año. Es, tras la dermatitis herpetiforme, la enfermedad ampollar pediátrica más frecuente. Ocurre en brotes con lesión patognomónica en collar de perlas y afecta preferentemente la zona genital y peribucal. Su diagnóstico se basa en una alta sospecha clínica y en la biopsia de piel con observación de ampollas subepidérmicas y depósito lineal de inmunoglobulina A en inmunofluorescencia directa. Frecuentemente, el diagnóstico es tardío debido al desconocimiento de esta enfermedad. Palabras clave: dermatosis bullosa por inmunoglobulina A lineal, impétigo, dermatitis herpetiforme.ABSTRACT Linear immunoglobulin A bullous dermatosis is a rare entity with frequent spontaneous resolution. It usually presents in children with average age of 4.5 years. Its incidence is about 0.5-2.3 cases/million individuals/year. It is, after dermatitis herpetiformis, the most frequent paediatric blister disorder. It usually appears in bouts with acute development of vesicles in strings of pearls; affecting the perioral area and genitalia. Diagnosis is based on the clinical signs and symptoms and biopsy of the skin with subepidermal blister and a linear band of immunoglobulin A in the direct immunofluorescence. Often, diagnosis is made late because of the unawareness of this disease.
