Limits of internal fixation in long-bone fracture

Abstract: Alternatives to internal fixation of long-bone fracture comprise, depending on location, external fixation or joint replacement. Limitations comprise risk of infection and functional outcome quality, which vary according to technique. The present study examines these limitations, based on comparative or large-scale studies from which certain significant results emerge. Four main questions are dealt with: (1) the present role of locking plates; (2) conditions for intramedullary nailing in Gustilo grade IIIb ope… Show more

“…Operative fixation of long bone fractures can reduce hospital stay, provide a quicker return to work, and improve fracture healing. External fixation devices, such as the Ilizarov frame, are favoured in low‐resource settings because of their ease of application and low complication rates compared with internal fixation methods.…”

“…6 There is a wide range of screw failure processes that can include screw backout, stripping, complete fracture or loosening due to infection. 3,7 Screw loosening is considered an unsolved issue and new techniques or devices that prevent loosening would represent a significant development for orthopaedic clinical practice.…”

“…[7] There are various new fixation models developed for this type of fractures including plate fixation, screws, sleds, absorbable implants and tension band wiring systems. [8][9][10] However, there is still no recognized gold standard for fixation of medial malleolus fractures. Kirschner wire (K-wire) fixation with a tension band construct, partially threaded K wire fixation, and partially threaded cancellous screws have been used for this purpose.…”
