Limited value for ultrasonography in predicting flare in rheumatoid arthritis patients with low disease activity stopping TNF inhibitors

Abstract: In RA patients with longstanding low disease activity, at time of stopping TNFi, US is a predictor for flare at group level, but at the patient level, US has limited added value when common clinical parameters are used already, though the predictive value of clinical predictors is modest as well.

“…In accordance with previously published data, we showed that PD synovitis may be a predictor of tapering failure 18,22,30 . In our study, patients with a US PD score ≥1 had significantly greater chances to relapse, indicating that power Doppler activity may be related to an incomplete suppression of inflammation, and, for this reason, patients without PD could be a more appropriate population for tapering DMARD.…”

“…This is likely a reflection of the high proportion of patients in our cohort who had ultrasound activity at baseline. In the POET study only 63% of patients had US sign of arthritis with positive power Doppler signal [30]. This is partly explained by our cohort, which included a greater proportion of patients, one third, with low disease activity state (LDAS) above the DAS-28 remission cut-off.…”

“…Lamers-Karnebeek, et al reported on MSK-US in a 1-year prospective study of patients with RA in low disease activity or remission who stopped antitumor necrosis factor agents. MSK-US could not predict flare on an individual level 13 . Thus, the 2 main reasons for MSK-US in patients with RA in remission are for prevention of damage from ongoing synovitis and prediction of flare, especially when medication de-escalation needs to be considered.…”
