Life-Threatening ACE Inhibitor-Induced Angioedema After Eleven Years on Lisinopril

Abstract: Angiotensin-converting enzyme inhibitors (ACE-Is) are the primary medication class implicated in drug-associated angioedema. Angioedema is most common early in ACE-I therapy, yet episodes can occur late in therapy and have been reported even as late as 10 years after single treatment initiation. We present a case of a 65-year-old African American woman who experienced 2 episodes of angioedema, with the second being life threatening after receiving several concomitant agents known to cause angioedema, most nota… Show more

“…A literature search using PubMed and the keywords “angiotensin‐converting enzyme inhibitor” and “angioedema” revealed multiple cases of ACEI‐induced angioedema that developed more than 1 year after the initiation of the drug. One case described the development of visceral angioedema after 9 years of enalapril therapy, while another described two episodes of angioedema occurring 11 years after the initiation of therapy with lisinopril . A separate case report noted the development of ACEI‐induced angioedema 23 years after the initiation of enalapril in a patient without any known risk factors, which, per our review, is the longest reported time from the initiation of ACEI therapy to the development of angioedema in the literature.…”

“…The development of ACEI-AAE can present itself within a few hours of the first ACEI intake, as in our case, and as late as after 20 years of regular use 9 10. With corticosteroid and antihistamine treatment, the mean time until complete resolution is 27.1 hours, and with the use of icatibant (a specific bradykinin receptor antagonist), 8.0 hours 6.…”

“…A number of cases of ACEi-induced angio-oedema have been successfully treated with complement C1-esterase inhibitor concentrate, which in Denmark is approximately 25% cheaper per dose compared to icatibant (ie, Berinert 1000 U=€1400) 8 9 16 17…”
