Lichen planus hypertrophicus of the vulva – a rare entity

Abstract: Vulvovaginal lichen planus is a type of chronic inflammatory dermatosis of the vulva, usually seen as a part of widespread lichen planus. The common clinical types of vulval lichen planus include papulosquamous, erosive and pigmented types. Hypertrophic lichen planus of the vulva is a rare entity. We report a case of hypertrophic lichen planus of the vulva which clinically simulated genital warts.

“…Hypertrophic LP of vulvae is rare, and presents with hyperkeratotic white plaques involving vulvae, perineum, and perianal area, which sometimes gives an appearance similar to squamous cell carcinoma (SCC) of vulva with incidence seen in 3% of cases. [ 7 ] Hypertrophic LP of vulva may be associated with HCV, autoimmune disorders, alopecia areata, and coeliac disease. [ 7 ] In lichen planopilaris,[ 4 ] follicular keratotic papules are limited to the hair-bearing labia majora and mons pubis but can involve scalp, trunk, and extremities.…”

“…Опыт лечения гипертрофического КПЛ вульвы ограничен, что связано с редкостью патологического процесса. Лишь в одном сообщении указывалось на эффективность при этом заболевании внутриочаговых инъекций кортикостероидных гормонов [14].…”

“…[ 3 ] and Job et al . [ 4 ] In the publication by Mahajan et al .,[ 3 ] hypertrophic vulval LP presented as a soft, well-defined hyperpigmented plaque elaborating multiple follicular openings that covered almost the entire mucosal aspect of the right labia majora. Job et al .,[ 4 ] however, demonstrated multiple, asymptomatic, grouped, coalescing hyperpigmented fleshy papulonodules on the vulva, simulating genital warts as the hallmark finding.…”
