Leydigzell-Tumoren des Hodens - klinische und morphologische Aspekte

Abstract: Leydig cell tumors are the most common tumors of the gonadal stroma. They account for 3% of all testicular tumors. Whereas in 10% of adult cases malignancy occurs, the clinical course in children is benign. During childhood the tumor presents with precocious pseudopuberty, in adults with testicular swelling and gynecomastia. We report on 17 cases of Leydig cell tumor, paying special attention to clinical and morphological aspects.

“…Leydig cell tumors are normally associated with endocrine disturbances due to secretion from Praz/Strebel/Regusci/Sarazin/Hauri the tumorous cells. In children, the peak incidence is between the ages of 5 and 10 years and owing to autonomous production of testosterone, the tumors lead to precocious puberty, producing masculinization [8][9][10]. In adults, they manifest between 30 and 35 years of age, in the form of feminization, consisting of unilateral or bilateral gynecomastia, diminution of the libido, impotence and contralateral testicular atrophy [5,8,9,11].…”

“…This kind of tumor is mostly benign. However, approximately 10% of the cases, and uniquely in adults [5][6][7][8][9], demonstrate malignant behavior, presenting with metastatic disease. “…Masur u. Mitarb. berichteten über eine Serie von insgesamt 17 Patienten mit benignen Leydig-Zelltumoren und beobachteten bei 57,1 % erhöhte Serumöstradiolspiegel in der Vena spermatika der betroffenen Seite, während die periphere Blutanalyse dies nur in 28,5 % der Fälle bestätigte [9]. Hyperöstrogenismus beeinflusst die Hypothalamus-Hypophysen-Gonadenachse auf verschiedenen Ebenen.…”
