InleidingEen slechte mondgezondheid kan een negatief effect hebben op iemands levenskwaliteit (Verrips, 2011). Zo kunnen mensen dagelijks last hebben van pijn, problemen met eten of zich schamen voor hun gebit. Aan de schaamte kan in de meeste gevallen iets worden gedaan door middel van een orthodontische behandeling. Het blijft echter de vraag of een orthodontische behandeling werkelijk iemands levenskwaliteit kan bevorderen. Het is niet eenvoudig deze vraag te beantwoorden doordat de populatie van orthodontiepatiënten voor het merendeel uit kinderen en adolescenten bestaat. Deze patiënten doorlopen tijdens de langdurige orthodontische behandeling psychologische stadia die invloed hebben op hun levenskwaliteit, bijvoorbeeld de puberteit en het eerste contact met de middelbare school. Ook de craniofaciale groei beïnvloedt de levenskwaliteit (Prahl en Bos, 2011). Het effect van deze factoren verschilt per patiënt.Er zijn in de afgelopen jaren verschillende vragenlijsten voor kinderen en adolescenten ontwikkeld. Een van de meest gebruikte is het Child Oral Health Impact Profile ( COHIP). Deze is ontworpen door een internationaal team van onderzoekers . Er bestaan 2 versies, een voor kinderen van 8 tot en met 15 jaar oud en een voor hun ouders. In Nederland is de COHIP in de afgelopen jaren getest en verder ontwikkeld (Geels et al, 2008a;Geels et al, 2008b). De huidige versie bestaat uit 30 vragen over de relatie tussen mondgezondheid en levenskwaliteit, verdeeld over 5 subschalen: symptomen aan het gebit, functioneren van het gebit, emotioneel welbevinden, welzijn op school, en interactie met vrienden.Het doel van dit onderzoek was te bepalen hoe kinderen die een orthodontische behandeling ondergaan, en hun ouders aankijken tegen de levenskwaliteit van het kind en of er in dit verband verschillen zijn tussen de levenskwaliteit van jongens en meisjes.
Materiaal en methodenAan het onderzoek deden 182 orthodontiepatiënten en hun ouders mee. De gemiddelde leeftijd van de orthodontiepatiënten was 12 ± 1,5 jaar en er waren evenveel jongens als meisjes. In het vervolg van dit artikel worden de orthodontiepatiënten aangeduid als 'kinderen'. Door zowel de ouders als de kinderen werd de COHIP-vragenlijst ingevuld. Daarnaast beantwoordden zij enkele exploratieve vragen die beoogden inzicht te geven in het welzijn van de kinderen. Deze vragen staan vermeld in tabel 2.Scores op de subschalen en de totale COHIP werden vergeleken met behulp van t-testen, correlaties tussen subschalen werden geanalyseerd. Met een regressieanalyse werd gekeken naar de voorspellende waarde van de subschalen en de totale COHIP op de algemene gezondheidsbevinding van ouders en kinderen.
ResultatenDe scores van de ouders en de kinderen bleken sterk overeen te komen. De kinderen gaven echter wel vaker dan hun ouders aan dat zij bloedend tandvlees, pijn, een droge mond Een 14-jarige jongen die wordt behandeld met volledig vaste apparatuur.
