Levator Plication versus Resection in Congenital Ptosis – A Prospective Comparative Study

Abstract: In 70% cases of congenital ptosis, good cosmetic outcome can be achieved with levator resection. With levator plication there are greater chances of drooping from fourth week onwards. The technique of plication, though simpler in approach failed to correct dystrophic muscle in congenital ptosis.

“…In the study by Kumar et al, 11 with plication of the levator aponeurosis using absorbable sutures, the upper eyelid continued to fall after the fourth week until the final follow-up visit at 3 months. This drooping of the lid postoperatively was not observed in cases of levator resection.…”

“…Since then, the levator aponeurosis plication has attracted extensive attention. However, this technique did not always achieve satisfactory correction result because of the insufficient understanding of the anatomical structure of the upper eyelid and too simple surgical procedure in the past 14–17 . During the last 2 decades, the precise and complex anatomical structure of upper eyelid has been further understood 18–22 .…”

“…Levator aponeurosis plication was first described by Eversbusch 12 1883 and was reintroduced and redefined by Jones et al 13 structure of the upper eyelid and too simple surgical procedure in the past. [14][15][16][17] During the last 2 decades, the precise complex anatomical structure of upper eyelid has been further understood. [18][19][20][21][22] Based on these changes in the understanding of the upper eyelid structure, our team improved the levator aponeurosis plication in 3 aspects: dissection, fixation, and closure of incision.…”

“…Kumar et al have compared the results of levator resection versus placation in congenital ptosis, where they found superior results in levator resection group 6. Thus, if the plicated levator bulk causes tarsal eversion and conjunctival prolapsed under such circumstances levator resection can be considered.…”
