На настоящий момент оценка состояния головного мозга ребёнка, имеющего перинатальное его поражение, проводится в основном по имеющейся симптоматике и данным лабораторных тестов. Из аппаратных диагностических методик применяются лишь нейровизуализационные-МРТ, КТ и нейросонография. Простые и информативные нейрофизиологические тесты, несмотря на многолетнюю историю их применения, используются недостаточно. С целью объективизации состояния нейрональных и проводящих структур головного мозга проводились МРТ и исследования стволовых вызванных потенциалов на акустическую стимуляцию 29 детям (10 мальчиков, 19 девочек) с перинатальными поражениями головного мозга различной природы в периоде новорожденности. Во всех случаях изменения структуры мозговой ткани на МРТ сопровождались изменением характеристик нейрофизиологического сигнала. При наличии МРТ-указаний на нарушение структуры миелинизированных образований значимое увеличение временных параметров вызванных потенциалов отмечено лишь в единичных случаях (у 4 детей). В 25 случаях изменению подвергались амплитудные и конфигурационные параметры стволового ответа. Тем самым, использование в качестве основного источника информации временных параметров стволового вызванного потенциала при проведении исследований детям первых месяцев жизни нецелесообразно. Наиболее информативными и коррелирующими с данными нейроимиджинговых исследований являются такие параметры, как общий амплитудный уровень кривой стволового ответа и расщепление его отдельных компонентов.

Fatal Congenital Lactic Acidosis in Two Siblings

Summary Two siblings have been described who developed a fatal metabolic acidosis during the first days of life. The acidosis was found to be caused by an accumulation of lactic acid in the second sibling. In the first sibling, no determination of lactic acid was performed, but the similarities in clinical, biochemical and pathological findings combine very strongly to suggest an identical condition. The children died at the age of 100 and 177 hours. The main pathological findings were confined to the brain, the thymus and the liver. Softening of the white matter was pronounced with delayed myelination. Organized bilateral paraventricular cystic spaces were found. Fatty changes were pronounced in the liver. In some respects these children differ from those previously reported with congenital lactic acidosis.
