Leukocyte-Rich versus Leukocyte-Poor Platelet-Rich Plasma for the Treatment of Knee Osteoarthritis: A Double-Blind Randomized Trial

Abstract: Background: Platelet-rich plasma (PRP) is gaining large interest in clinical practice as a minimally invasive injective treatment for knee osteoarthritis (OA). Different preparation methods are available, and the presence of leukocytes, deemed detrimental in some preclinical studies, is one of the most debated aspects regarding PRP efficacy. Purpose: To compare the safety and effectiveness of leukocyte-rich PRP (LR-PRP) and leukocyte-poor PRP (LP-PRP) for the treatment of knee OA. Study Design: Randomized cont… Show more

“…This effect may be attributed to the presence of leukocytes, which could promote an initial inflammatory reaction [ 36 , 37 , 38 , 39 , 40 ]. Although the final overall balance is an anti-inflammatory effect thanks to the inhibition of the NF-kB pathway [ 41 ], as also demonstrated in other clinical studies about knee injective therapy [ 42 ], leukocytes could cause an initial pain reaction. This, however, is self-limiting and seems not to affect the final treatment outcome [ 43 ].…”

“…The PRP injection therapy applied to the knee joints of patients with early-stage primary OA seems to be an effective treatment method for pain control and improvement of function in treated patients, as reported in various studies [ 12 - 15 ]. On the other hand, alternative treatment options for patients who are resistant to PRP injection are limited.…”

“…Both PRP and ACS are the treatment methods that attract the attention of physicians and patients. For this reason, many studies on PRP and ACS treatment methods have been published and continue to be published [ 12 - 19 ]. This study aims to understand the effects of the two new and popular treatment methods i.e., ACS and PRP, on pain and clinical scores in OA treatment.…”

“…In the comparative literature (Di Martino et al, 2022;Krief et al, 2021;Zhang, 2021), studies consider the safety of the method through the rate of adverse effects, whereas in our work we objectively focus on the absence of contaminants, being this paper the first report with this type of analysis.…”
