The invasion of the male reproductive tract by microorganisms, and its subsequent consequences for sperm fertilizing potential, has been intensely discussed. The role of the bacteria that are responsible for the colonization and contamination of the male urogenital tract, rather than its infection, in diminished sperm parameters raises the most controversy. There are numerous premises suggesting that bacterial semen infection is associated with male infertility. However, the molecular mechanism by which the fertility is affected is complex and multifactorial, and still presents a puzzle. Some authors have suggested that direct interactions between bacteria and human spermatozoa facilitate sperm immobilization, affect sperm morphology, and thus weaken the ability of sperm to fertilize. On the other hand, the massive infiltration of activated leukocytes into the inflammatory site may be associated with impairment of sperm fertilizing potential, due to oxidative, apoptotic, and immune processes. This review presents current research trends and aims to summarize the present knowledge of semen inflammation and causative bacterial agents in the male urogenital tract, with its consequence on seminological parameters, and male fertility status.

BackgroundIt is not well established whether the increased number of leukocytes in the seminal fluid impairs the outcomes of assisted reproductive technology (ART). This investigation analysed the outcomes of the intracytoplasmic sperm injection (ICSI) and intracytoplasmic morphologically selected sperm injection (IMSI) cycles in couples in which the male partner exhibited leukocytospermia.MethodsA total of 100 cycles in 100 couples were included in this study. For the ICSI or IMSI procedures, the patients were divided into two groups according to the presence or absence of leukocytospermia and then matched by (female) age: ICSI: Group I (n = 25): Leukocytospermia - semen samples with a leukocyte count of greater than or equal to 1 × 10(6)/mL; and Group II (n = 25): Non-leukocytospermia - semen samples with a leukocyte count < 1 × 10(6)/mL. IMSI: Group I (n = 25): Leukocytospermia; and Group II (n = 25): Non-leukocytospermia.The endpoints included the rates of fertilisation, implantation, clinical pregnancy, miscarriage, ongoing pregnancy and live birth. Student’s t-tests, Mann–Whitney tests and Chi-square tests were performed, and P < 0.05 was considered significant.ResultsThe data from the ICSI groups showed that leukocytospermia did not have a negative influence on the rates of fertilisation (Group I: 57.9+/−30.2%, Group II: 61.9+/−27.7%; P = 0.74), implantation (Group I: 12.3%; Group II: 13.5%; P = 0.93), clinical pregnancy (Group I: 24%; Group II: 24%; P = 1.0), miscarriage (Group I: 0, Group II: 0), ongoing pregnancy (Group I: 24%; Group II: 24%; P = 1.0), or live births (Group I: 24%; Group II: 24%; P = 1.0). Similarly, the data from the IMSI groups also showed that the leukocytospermia did not have a negative influence on the rates of fertilisation (Group I: 67.6+/−24.6%, Group II: 59.5+/−28.1%; P = 0.36), implantation (Group I: 17.5%; Group II: 16.7%; P = 0.90), clinical pregnancy (Group I: 28%; Group II: 24%; P = 1.0), miscarriage (Group I: 14.3%; Group II: 0; P = 0.33), ongoing pregnancy (Group I: 24%; Group II: 24%; P = 1.0), or live births (Group I: 24%, 6/25; Group II: 24%, 6/25; P = 1.0).ConclusionsThe results indicate that the leukocytospermia may not have a negative effect on the outcomes of ICSI or IMSI cycles. Nevertheless, it seems that it is necessary to more precisely determine the effects, if any, of seminal leukocytes on fertilisation and implantation processes. Such efforts will help to establish a more reliable leukocyte threshold, which could eventually demonstrate whether there is a negative influence on the ART procedures.

Oosit toplama günü semen örneklerinde rastlantısal olarak izole lökospermi saptanan olguların in vitro fertilizasyon / intrasitoplasmik sperm enjeksiyonu (İVF/İCSİ) tedavi sonuçlarını değerlendirmek. Gereç ve yöntem: Akdeniz Üniversitesi Tıp Fakültesi Üremeye Yardımcı Tedavi Merkezi'nde, Ocak 2015 ve Mart 2016 tarihleri arasında, İVF/İCSİ tedavisi uygulanmış hastalara ait bilgiler elektronik veri tabanı kullanılarak gözden geçirildi. Oosit toplama günü yapılan incelemede izole lökospermi (≥1x10(6)/mL) saptanan hastaların (n=70) belirlenmesinin ardından, indeks olguya en yakın dönemde oosit toplama işlemi gerçekleştirilmiş; kadın yaşı, metafaz 2 (M2) oosit ve önceki tedavi sayıları eşleştirilmiş ancak lökospermi saptanmayan (˂1x10(6)/mL) hastalar (n=70) seçilerek kontrol grubu oluşturuldu. Fertilizasyon oranları, embriyo kaliteleri ve gebelik sonuçları gruplar arasında karşılaştırıldı. Bulgular: Gruplar arasında M2 oosit ve 2-pronuclei ortalamaları benzer bulundu (p>0.05). Ancak fertilizasyon oranının lökospermi grubunda daha düşük olduğu saptandı (%61.4 ve %69.7, p=0.012). Klivaj evresi (2-3. gün) yüksek kalite embriyo sayıları ve fertilize oositten yüksek kalitede klivaj embriyo gelişim oranı benzer bulundu (p>0.05). Hasta başına yüksek kalitede blastokist sayısı ve yüksek kaliteli klivaj evresindeki embriyolardan yine yüksek kaliteli blastokist gelişim oranında istatistiksel farklılık saptanmadı (p>0.05). Buna karşılık, total fertilizasyon başarısızlığının (%14.3 ve %2.9, p=0.016) ve siklus iptallerinin (%34.3 ve %17.1, p=0.020) lökospermi grubunda daha sık olduğu saptandı. Gebelik sonuçları incelendiğinde, kontrol grubunda siklus başına pozitif beta-insan koryonik gonadotropin (β-hCG) oranı daha yüksek bulunmasına karşın (p=0.033), embriyo transferi başına pozitif β-hCG ve klinik gebelik oranlarında gruplar arasında istatistiksel olarak anlamlı fark izlenmedi (p>0.05). Sonuç: Lökospermi fertilizasyonu etkileyebileceğinden total fertilizasyon başarısızlığına ve siklus iptallerine neden olabilir. Ancak, fertilizasyonun sağlıklı şekilde gerçekleşmesi halinde embriyo kalitesinin ve gebelik sonuçlarının olumsuz etkilenmediği söylenebilir.
