Letter to the Editor: Thoraco-Acromial Artery Perforator (Taap) Flap for Reconstruction of a Recurrent Dermatofibrosarcoma Protuberans of the Clavicular Region

Abstract: Dermatofibrosarcoma Protuberans is a rare mesenchymal tumor, highly infiltrative and with a high recurrence rate. Despite its localization may vary, the trunk is widely recognized as the most common interested site. We read with interest the recent article published by Longo et al. "Recurrent dermatofibrosarcoma protuberans of the clavicular region: radical excision and reconstruction with Latissimus Dorsi myocutaneous flap"; since their case is very similar to one we faced in 2016. In this letter, we present … Show more

“…Since its introduction, the FALD flap received outstanding attention because it represents a totally autologous reconstruction, is easy to harvest, and does not need microsurgical experience. [22][23][24] With the proper patient selection, this flap can guarantee a pleasant aesthetic outcome and adequate breast volume in a single operative session. 25 As a result, the FALD flap has become increasingly popular and utilized.…”

“…However natural anti-E autoantibodies may appear in patients with no known history of transfusion, nor pregnancy 17 . Given the severity and the extent of the defect, a free flap reconstruction was indicated [18][19][20][21] . The patient was informed on the risks inherent to the procedure and his hypercoagulable state and accepted to undergo the surgery.…”

“…Nevertheless, there do exist more reconstructive options which are indicated to patients with high-risk comorbidities 4 , contraindications to microsurgery 5,6 , and either insufficient or inappropriate abdominal tissue for DIEP flap 7 . FALD flap represents a suitable alternative in such cases as, being a myocutaneous flap 8 , it offers a huge availability of soft tissues and gives the plastic surgeon the possibility of a total autologous reconstruction of small to moderate sized breasts without implants [9][10][11][12][13] . The latissimus dorsi (LD) muscle, muscular component of FALD flap, is a wide triangular structure originating on the spine and ilium and extending across the upper and mid-back to insert on the humerus 14 .…”
