Lethal Tuberculosis in a Previously Healthy Adult with IL-12 Receptor Deficiency

Abstract: A 33-year-old man was admitted in hospital due to fever, generalized lymphadenopathy, and hepatosplenomegaly. He had a history of anti-tuberculosis treatment in the previous 3 years. Despite normal chest radiograph, a sputum sample was smear-positive for acid-fast bacilli, and polymerase chain reaction was positive for Mycobacterium tuberculosis complex. Drug susceptibility test revealed resistance to isoniazid and rifampin. Evaluation of the patient's immune system revealed IL-12Rβ1 deficiency. The patient di… Show more

“…[1][2][3][4] Infections generally occur in children, with a wide range of clinical symptoms, from localized to disseminated infections. [1][2][3] The patients are also vulnerable to the more virulent Mycobacterium tuberculosis, [5][6][7] and about half of them also suffer from nontyphoidal Salmonella infection. 8,9 Other infections are increasingly documented, albeit typically in individual patients.…”

“…These observations raise fundamental questions about the mechanisms underlying incomplete penetrance and variable expressivity that have yet to be answered. In this context, we serendipitously discovered the first cases (to our knowledge) of monogenic tuberculosis, in patients and even in families with IL-12Rβ1 deficiency manifesting solely as severe tuberculosis (86)(87)(88)(89)(90)(91). These patients were normally resistant to live bacillus Calmette-Guérin vaccine and environmental mycobacteria.…”

“…Inversely, infections with various salmonella can occur repeatedly. Infections with other pathogens such as candida [Cardenes et al, 2010; de Beaucoudrey et al, 2010] and, occasionally, M. tuberculosis have also been observed [Caragol et al, 2003; Özbek et al, 2005; de Beaucoudrey et al, 2010; Tabarsi et al, 2011; Boisson-Dupuis et al, 2011]. …”
