Letalidade por doenças isquêmicas do coração no Estado do Rio de Janeiro no período de 1999 a 2003

Abstract: There has been a progressive increase in the number of CABG and PTCA procedures to treat IHDs in the RJ. Lethality rates were above the desirable level, mainly for chronic IHD hospital admissions (5.4% and 1.7%, respectively). Optimized medical therapy appears to be a worthwhile therapeutic option, reserving CABG and PTCA procedures for the cases with the worst prognoses. Lethality rates for AMI with medical therapy was comparable to current rates when thrombolytics were not used (16.7%).

“…Another issue to be considered in the increase mentioned refers to the technological advances in diagnostic and interventional treatment for this disease (coronary artery bypass and angioplasty) which occurred in recent years. According to Oliveira et al 29 , in a study conducted in the state of Rio de Janeiro, there was a significant increase in admissions for IHD, with a concomitant increase in high-complexity procedures in admissions.…”

“…In the analysis of decreasing mortality trends due to IHD, multiple factors must be considered, including age, sex, diagnosis, therapeutic interventions and environmental and socioeconomic conditions. The studies with data recorded from clinical practice provide a view that is closer to reality than the experimental ones 3 .…”

“…Regarding the comments made by reader(s) of the article "Letalidade por doenças isquêmicas do coração no estado do Rio de Janeiro no período de 1999 a 2003" ("Lethality due to ischemic heart diseases in the State of Rio de Janeiro, from 1999 to 2003") 1 , we would like to clarify the following: in the three fi rst paragraphs of the discussion we listed all the limitations of the database used for the study, recognizing the need for more specifi c information on the clinical status of patients, comorbidities and risk factors that would allow us to develop more accurate adjustment scores.…”
