Lessons learned from studies of psychotherapy for posttraumatic stress disorder via video teleconferencing.

Abstract: This article summarizes two ongoing randomized controlled trials that compare individual in-person psychotherapy with psychotherapy provided using video teleconferencing for military veterans with posttraumatic stress disorder. We describe training methods, populations, technology, challenges, successes, and lessons learned so far during the trials.

“…Very few published randomized trials have evaluated the effectiveness of web-based videoconferencing interventions, though several new studies are currently underway (e.g., Strachan et al, 2012; Thorp et al, 2012). In addition, available studies have not explored potential unintended negative effects of web-based delivery of routine counseling that might compromise acceptability in and transportability to community-based programs.…”

“…In addition, available studies have not explored potential unintended negative effects of web-based delivery of routine counseling that might compromise acceptability in and transportability to community-based programs. These effects include the impact of web-based care on patient-therapist relationships, and technological obstacles that might interfere with the expected convenience of the intervention (e.g., Sucala et al, 2012; Thorp et al, 2012; Deen et al, 2013; Shore, 2013). …”

“…38,39 Previous accounts of CV-delivered exposure therapy make several helpful, experientially based suggestions for implementation. 12,13,36 We hope to continue to build on this experiential knowledge to further the clinical utility of CV-delivered exposure therapy intheater. Over the course of executing more than 50 CV encounters in a war zone during the first author's deployment, the following realizations became evident: (1) Given the high amount of turnover and transition among providers within and between deployments, it is imperative that all providers using CV technology be briefed prior to, or at the beginning of, deployment.…”

“…36 In other words, video communication may not have just facilitated an effective replication of face-to-face services, but may have imparted a helpful degree of interpersonal distance from the therapist for self-disclosure and engagement. Although the argument that active duty individuals deployed to a combat zone may be especially guarded seems to have face validity and there is evidence that deployed individuals are biased to minimize symptoms in general, 4 there is insufficient evidence to conclude that CV modalities are, in fact, related to greater disclosure.…”

“…Ron Acierno, Leslie Morland, Peter Shore, and Peter Tuerk) so that we can exchange best practices regarding PTSD assessment and treatment and the provision of services via videoconferencing. Our two publications on this topic (Backhaus et al, 2012 -Thorp is corresponding author; Thorp et al, 2012) continue to generate citations, reflecting their utility in this field. VA Central Office (VACO) asked the PI to participate on the Committee to Develop the Appendix to the VHA Telemental Health Operations Manual for Delivery of Evidence-Based Psychotherapy for PTSD.…”
