“…These features can lead to misdiagnoses and grade CCS as nonaggressive lesions (74). On T1w, most CCS (about 52%) have a slightly increased SI compared with muscle, while they are hyperintense to muscle on T2w (67, 74). Because of paramagnetic effects of intralesional melanin, high SI on T1w and lower SI on T2w should theoretically be present (74).…”

“…Areas of more prominent adipose tissue have also been described at the poles of the lesion and they appear as a hyperechoic triangular cap, termed “the bright cap sign” (63). On MRI, IM presents as a well‐circumscribed, homogeneous mass with SI lower than or equal to muscle on T1w and brighter than fat on T2w (67). Myxomas show an intense heterogeneous contrast‐enhancement, although they are composed of bland spindle cells with an avascular highly myxoid stroma; moreover, myxomas are poorly vascularized, typically with capillary‐like vessels (Fig.…”

“…Since hyperintensity on T1w (compared with muscle) is rarely seen in STS, it is quite an important finding to narrow down differential diagnoses (74). Hyperintensity on T2w could be caused by a low nucleo‐cytoplasmic index, by abundance of loose connective tissue between the cellular nests, or by high amount of myxoid stroma found in these hyperintense lesions (67). As with synovial sarcoma and epithelioid sarcoma, intimate association with a tendon may provide a clue to the diagnosis (Table 9) (71).…”
