Introduction: The purpose of this work is to evaluate the epidemiological, clinical and therapeutic aspects of spermatic cord in our context. Patients and Methods: This is a retrospective study over a period of 4 years from January 1, 2014 to June 30, 2018. Results: We collected 55 cases divided into 26 cases, 44.27% for the Thies Regional Hospital, 21% or 38.18% for the Mbour EPS and 8 cases representing 14.55% for the Saint Jean de Dieu Hospital. We found an average age of 20.03 years with extremes of 4 years and 56 years. In 28 patients or 50% of cases, we found no particular pathological history, while in 4 patients or 7.14%, we found a notion of recurrent ipsilateral testicular pain. We recorded 67.27% (n = 37) of the cases received for testicular pain; 15 patients or 27.27% presented a large painful pouch. Two patients (3.64%) were received for inguinal swelling and 1 patient (1.82%) presented testicular atrophy. The average consultation time is 16.9 hours with extremes of 2 hours and 96 hours. The torsion was in 59.61% of cases (n = 32) located on the right versus 40.38% (n = 22) on the left. There was no bilateral form. The pickup time was 3 hours with extremes of 1 h to 24 h. All patients benefited from an exploratory scrotomy. We found a total of 22 patients or 40% of cases with ischemic testis, 8 cases or 14.55% of patients with necrotic testis and 2 cases or 3.64% where the testis was normal. Orchidopexy was performed in 46 patients or 83.63% and orchiectomy in 9 patients. Conclusion: Care is urgent. There is a need for better awareness of the population and the medical staff for the early diagnosis.

IntroductionLes grosses bourses aiguës (GBA) sont des motifs fréquents de consultation en urgence. Cette étude vise à déterminer la fréquence hospitalière des GBA au Centre Hospitalier Régional Amadou Sakhir Mbaye (CHRASM) de Louga au Sénégal, à identifier les formes cliniques et à évaluer les modalités de prise en charge.MéthodesIl s’agit d’une étude rétrospective, descriptive portant sur 114 patients reçus pour GBA au CHRASM de Louga, de mai 2010 à août 2013. Les variables étudiées étaient: la fréquence des GBA parmi les grosses bourses (GB) et les urgences urologiques, l’âge, le délai de consultation, les causes, le traitement, l’évolution post-thérapeutique et la durée d’hospitalisation.RésultatsAu cours de la période d’étude, 356 GB et 420 urgences urologiques ont été enregistrées. Ainsi, les 114 cas de GBA recensés représentaient 32,0 % des GB et 27,1 % des urgences urologiques. La moyenne d’âge était de 42,25 ± 25ans (extrêmes de 5 mois et 89 ans). Le délai moyen de consultation était de 4 jours. Le diagnostic évoqué à l’admission était: une orchi-épididymite aiguë (n=66), une GBA abcédée (n=18), une suspicion de torsion du cordon spermatique (n=14), une GBA traumatique (10 cas), une hernie inguino-scrotale étranglée (06 cas). Le traitement a été exclusivement médical dans 66 cas (57,8 %). L’exploration chirurgicale qui était indiquée chez 48 patients, a été effectuée chez 45 d’entre eux (93,7 %), trois patients (6,2 %) l’ont refusé. Au total, il y a eu 9 cas d’orchidectomies et 36 procédures conservatrices. La durée moyenne d’hospitalisation était de 3± 2 jours. Quatre vingt et un patients (71,0 %) ont été hospitalisés pendant 24 heures au moins; ils étaient répartis en 42 patients chirurgicaux et 39 patients médicaux. Aucun cas de décès n’a été enregistré.ConclusionLes GBA sont fréquentes dans notre structure, tous les âges sont concernés. Elles sont dominées par les GBA d’origine infectieuse, les torsions du cordon spermatique et les traumatismes des bourses. Le retard à la consultation est souvent la règle, ce qui met en jeu le pronostic fonctionnel du testicule.

Introduction Spermatic cord torsion (SCT) is a rare urological emergency that can jeopardize the fertility of the patient. Our study aimed to investigate the epidemiological, diagnostic and therapeutic aspects of SCT in Souro Sanou University Teaching Hospital. Materials and Methods This was a descriptive cross-sectional study with retrospective data collection from January 1, 2017, to December 31, 2021, in the emergency surgical and urology division of Souro Sanou University Hospital. Results The annual frequency of SCT was 4.4 patients/year. The mean age of the patients was 17.82 ± 6.74 years. Painful swelling of the hemi-scrotum was the main presenting symptom. The torsion involved the left spermatic cord in 50% of cases and both spermatic cords in 5% of cases. The mean time of symptoms before presentation was 78.8 hours ± 153 hours. Only 27.3% of patients presented before the 6th hour after the onset of pain. The average time from presentation to surgical intervention was 5.6 hours ± 5 hours. Orchiectomy was performed in 11 patients (47.8%). The average hospital stay was 3.1 ± 1.4 days. Conclusion SCT is a rare urological emergency. The prognosis depends on the duration and degree of torsion. A high orchidectomy rate can be observed with a delay in presentation.
