We determined whether persistent nausea and vomiting (N/V) symptoms following Roux-en-Y gastric bypass surgery is due to elevated systemic glucagon-like peptide-1 (GLP-1) and leptin in female non-diabetic subjects. Subjects with N/V post-Roux-en-Y gastric bypass (RYGB) surgery had significantly elevated fasting GLP-1 levels compared to that with post-operative asymptomatic subjects and to morbidly obese, obese and lean subjects not undergoing surgery. Weight loss, glycaemia, insulin and post-prandial GLP-1 levels were similar in all postoperative subjects. Despite comparable BMI, leptin was significantly lower in symptomatic subjects. Furthermore, leptin secretion from subcutaneous adipose tissue was inhibited by GLP-1 (0.1-1.0 nM; n=6). Persistent N/V following RYGB surgery is associated with elevated fasting GLP-1, but lower leptin levels. The latter may be a consequence of the direct GLP-1 inhibition of leptin secretion from adipose tissue.

New optically active poly(ester-imide)s (PEIs) containing bicyclo[2,2,2]oct-7-ene-2,3,5,6-tetracarboxylic diimide in the main chain were prepared by the reaction of N,N'-(bicyclo[2,2,2]oct-7-ene-2,3,5,6-tetra carboxylic)bis-L-valine 4 as a diacid monomer with various aromatic diols 5a-g in a system of tosyl chloride (TsCl), pyridine (Py), and N,N-dimethylformamide (DMF). The reactions with TsCl were significantly promoted by controlling alcoholysis with diols, in the presence of catalytic amounts of DMF, to give a series of optically active poly(ester-imide)s. The resulting new polymers were obtained in good yields and inherent viscosities, ranging between 0.36 and 0.62 dL g −1 , and were characterized with FT-IR and 1 H-NMR spectroscopy, including specific rotation. All of these polymers were highly organosoluble at room temperature in solvents like N,N-dimethylacetamide (DMAc), N,N-dimethylformamide (DMF), dimethylsulfoxide (DMSO), N-methyl-2-pyrrolidone (NMP), and sulfuric acid. The thermal stability of the resulting PEIs was evaluated with thermogravimetric analysis (TGA) and differential scanning calorimetry (DSC) techniques under a nitrogen atmosphere, which indicated that the residual weight percent of the polymers at 600 • C was 43.69%-60.5% and showed that they are moderately thermally stable.

Apart from the common causes of thyrotoxicosis, such as Graves' disease and functioning nodular goiters, there are more than 20 less common causes of elevated free thyroid hormones that produce the symptoms and signs of thyrotoxicosis. This review describes these rarer conditions and includes 14 illustrative patients. Thyrotropin and free thyroxine should be measured and, when the latter is normal, the free triiodothyronine level should be obtained. Measurement of the uptake of 123 I is recommended for most patients.
