Leprosy reactions: The predictive value of Mycobacterium leprae-specific serology evaluated in a Brazilian cohort of leprosy patients (U-MDT/CT-BR)

Abstract: BackgroundLeprosy reactions, reversal reactions/RR and erythema nodosum leprosum/ENL, can cause irreversible nerve damage, handicaps and deformities. The study of Mycobacterium leprae-specific serologic responses at diagnosis in the cohort of patients enrolled at the Clinical Trial for Uniform Multidrug Therapy Regimen for Leprosy Patients in Brazil/U-MDT/CT-BR is suitable to evaluate its prognostic value for the development of reactions.MethodologyIgM and IgG antibody responses to PGL-I, LID-1, ND-O-LID were … Show more

“…IgG anti-LID-1 (LID-1-lot: November 14, 2011, Dr. Duthie, USA) and IgG and IgM anti-ND-O-LID (ND-O-LID-lot: August 17, 2012, Dr. Duthie, USA) were detected by ELISA as described by Hungria et al [31]. For anti-LID-1 and anti-ND-O-LID serology, the cutoff was determined by the ROC curve analysis using samples of leprosy cases and samples from EC.…”

“…Thus anti-LID-1 serology may be used as a predictive marker for the development of ENL, with a sensitivity of 71% and a specificity of 80%. [ 78 ] CCL-11 CCL-11, produced by monocytes, is a known chemoattractant for eosinophils and Th2 lymphocytes. It has been found to be a potential plasma marker for ENL.…”

“…Previous studies have investigated the potential use of leprosy serology as a marker of reactional episodes ( 50 – 52 ). Two previous publications from our group using the U-MDT database and sera bank ( 40 , 41 ) have described the impact of baseline serology on the development of leprosy reactions. The baseline ML flow test results showed limited sensitivity and specificity as prognostic markers for the development of leprosy reactions during subsequent follow-up ( 40 ).…”

“…The baseline ML flow test results showed limited sensitivity and specificity as prognostic markers for the development of leprosy reactions during subsequent follow-up ( 40 ). Also, the analyses of anti-PGL-I, anti-LID-1, anti-ND-O-LID antibodies at diagnosis showed low sensitivity and specificity for predicting reversal reaction while anti-LID-1 serology at diagnosis showed prognostic value for the development of ENL in BI positive patients ( 41 ). We acknowledge the importance of the analysis of the impact of reactional episodes on longitudinal serology data, however, these analyses are out of the scope of the current study which focused on the effect of different treatment regimens on leprosy serology to three antigens.…”
