Background: The disability in leprosy is caused by M. leprae invasion and infiltration into the skin and the mucous membrane that results in nerve damage and deformities, loss of sensation, paralysis, and ocular manifestation. The risk of disability can be influenced by several factors, including the type of leprosy, disease duration, the number of nerves affected, leprosy reactions, gender, age, type of treatment, socioeconomic factors, and leprosy case detection methods. This study aimed to evaluate the risk factor of disability in leprosy patients. Methods: It was a cross-sectional retrospective analytical study to discover the correlation between risk factors with the grade of disability in leprosy patients who were treated in Dermatovenerology Outpatient Clinic Dr. Soetomo General Hospital Surabaya from 2017 to 2019. Result: This study found a total of 275 leprosy patients with disabilities, consisting of 76 patients (27,6%) with grade-1 disability and 199 patients (72,4%) with grade 2 disability. There was a statistically significant correlation between age (p=0,025), duration of disease (p=0,001), and multidrug therapy (MDT) history (p=0,001) with the incidence of disability. Gender, bacterial index, type of leprosy, and leprosy reaction were not significantly related to disability. Conclusion: This study showed that age, duration of disease, and the history of MDT treatment were related to the incidence of disability. The longer the disease's duration despite the patient having received MDT, the more risk of the patient having a disability.

Abstract Papua and West Papua are two provinces in Indonesia with high rate of leprosy number. One of the problems in leprosy management is the emergence of leprosy reactions. The purpose of this study was to identified potential risk factors affecting to leprosy reaction among indigenous people in two leprosy pockets in Papua and West Papua. Cross-sectional studies were conducted in 10 months (February-November 2018) in Jayapura City and Bintuni District, by interview using questionnaire and physical examination. Successfully found 90 subjects of leprosy patients who are undergoing treatment and 18 subjects had experienced of leprosy reaction. Although no dominant potential risk factor was found, history of previous MDT consumption and history of BCG immunization become potential factors. Key words: reaction, leprosy, papua Abstrak Papua dan Papua Barat merupakan dua provinsi di Indonesia dengan angka kejadian kusta yang tinggi. Salah satu masalah dalam penanganan kusta adalah timbulnya reaksi kusta. Tujuan dari penelitian ini untuk mengidentifikasi faktor risiko potensial yang berpengaruh pada reaksi kusta pada etnis asli di dua kantong kusta di Papua dan Papua Barat. Penelitian potong lintang dilakukan selama 10 bulan (Februari-November 2018) di Kota Jayapura dan Bintuni, menggunakan wawancara dengan kuisioner dan pemeriksaan fisik. Berhasil ditemukan 90 subyek pasien kusta yang sedang menjalani pengobatan dan 18 di antaranya pernah mengalami reaksi kusta. Meskipun tidak ditemukan faktor risiko potensial yang dominan, namun riwayat minum MDT sebelumnya dan riwayat tidak diimunisasi BCG menjadi faktor-faktor yang potensial. Background: Bone health can be seriously affected by the leprosy reaction itself and steroids used in its treatment. Objective: To assess the bone mineral density (BMD) by dual-energy X-ray absorptiometry (DXA) scan and fracture risk by the fracture risk assessment tool (FRAX) in steroid treated patients with leprosy reaction. Methods: This study involved 25 patients with leprosy reaction treated with steroids (prednisolone) and 22 healthy family members attending the urban leprosy clinics of The Leprosy Mission International -Bangladesh (TLMI-B) Dhaka program in January-March, 2018. Socio-demographic and clinical information was recorded and BMD was measured in the spine, femur neck and forearm by DXA. Fracture risk was calculated by FRAX. Results: Both the T-score and Z-score of the spine were lower in the leprosy group in relation to those of the control group [leprosy vs. control: T-score −1.7 (−3.2-1.3) vs. −1.2 (−2.5-0.8), p =0.030 and Z-score −1.9 (−3.3 to −1.4) vs. −1.2 (−2.3 to −0.2), p =0.038; median (interquartile range). In other regions (femur neck, forearm) both scores were similar between the two groups, except for Z-score of the left femur neck which was lower in the leprosy group (p =0.036). Although the frequency of osteopenia and osteoporosis in different sites was similar between groups, the 10-year probability of major osteoporotic fracture [leprosy vs. control: 2.2% (1.6-4.7) vs. 1.1% (0.9-2.0); p <0.001] or hip fracture [leprosy vs. control: 0.4% (0.1-1.7) vs. 0.1% (0.0-0.2); p =0.001] by FRAX was higher in the leprosy group. Conclusions: Participants with leprosy reactions and steroid treatment had lower BMD related parameters in lumber spine and higher risk of osteoporotic fracture.
