Leprosy: An unusual presentation

Abstract: Leprosy is a chronic infectious disease caused by Mycobacterium leprae, commonly manifesting as hypopigmented anesthetic patches with thickened nerves. Primary hyperpigmentation in leprosy is a rare manifestation. Literature search showed only few case reports of Hansen's disease with single hyperpigmented patch and involvement of relatively immune zones like palms and soles. We report a case of leprosy presenting primarily as hyperpigmented patches over the trunk and extremities with involvement of palms and … Show more

“…1 Leprosy has a predilection for cooler and trauma prone areas of the body. 2 Ear and nasal septum are well recognised regions to be involved in leprosy owing to their cool temperature. Factors responsible for predilection of bacilli in these areas are because of their high ratio of surface area to tissue volume and distinctive pattern of vasculature as developed neonatally.…”

“…4 Lepromatous leprosy can present with varied manifestations like hyperpigmented patches, involvement of palms and soles, asymptomatic swelling over neck misdiagnosed as cervical lymphadenitis, granuloma annulare, erythema multiforme-like presentation, myositis, lymphadenopathy and so on. 2,[5][6][7] We report a case of lepromatous leprosy presenting as nodules over the nose mimicking rhinophyma.…”

“…The exact mechanism of primary hyperpigmentation in leprosy is unclear, and upregulation of melanogenic stimulating factors such as fibroblast growth factor, hepatocyte growth factor, and stem cell factors owing to neurohormonal or biochemical stimulations has been implicated. 11,12 The clinical presentation of the lesions on the palms and soles corresponded with the type of leprosy. The lesions in LL are numerically more in number, and this increases the chances of occurrence of lesions on the palms and soles also.…”
