Postdischarge Adverse Events Among Neonates Admitted to the Neonatal Intensive Care Unit

Objectives:We aimed to determine the rate of postdischarge adverse events (AEs), classify the types of postdischarge AEs, and identify risk factors for postdischarge AEs among neonates admitted to the neonatal intensive care unit (NICU).Study Design: This was a prospective cohort study of neonates admitted to the NICU from February 2017 through June 2019. We enrolled 170 neonates from a level 4 NICU who were being discharged home and whose parents can speak English and could be contacted after discharge. The main outcome of the study was postdischarge AEs based on structured telephone interviews, health record review, and adjudication by 2 blinded, trained physicians using a previously established methodology.Results: Fourteen percent of 170 neonates admitted to the NICU experienced postdischarge AEs, with 48% being either preventable or ameliorable. Adverse drug events and procedural complications comprised most of the AEs (48%), but most of the preventable and ameliorable AEs were due to management, therapeutic, or diagnostic errors. Seventy-nine percent of neonates who suffered an AE experienced either a readmission to the hospital or an emergency department visit. Neonates admitted to a level 4 NICU from another NICU (level 1, 2, or 3) (adjusted odds ratio, 3.62; 95% confidence interval, 1.27-12.60; P = 0.01) and those 28 to 36 weeks (adjusted odds ratio, 11.38; 95% confidence interval, 1.67-127.98; P = 0.01) had a significantly higher risk of AEs at discharge. Conclusions:Neonates discharged from a level 4 NICU were at high risk for experiencing postdischarge AEs. The identification of AE types and risk factors can be used to guide efforts to develop interventions to improve neonatal patient safety during the postdischarge period.

El objetivo de la investigación fue identificar y clasificar las especies de mariposas diurnas en el anexo 14, San Ramón, Junín, en el periodo comprendido entre los meses de junio y julio del 2022. La investigación fue de tipo aplicada de enfoque cualitativo y diseño no experimental donde se aplicó la técnica de observación y recolección de datos de distintas especies de mariposas nativas de la zona de estudio, para el cual fue necesario el trabajo de campo que permitió la aplicación del procedimiento de identificación y clasificación. Los resultados obtenidos representaron a 15 especies de mariposas tales como: Buho caligo illioneus, Anartia jatrophae, Anartia amathea, Methona themisto, Eurema daira, Morpho helenor, Morpho menelaus, Heliconius doris, Heliconius telesiphe, Mechanitis lysimniay, Greta oto, fueron las más representativas por la abundancia que se refleja en diversos puntos de muestreo. También, en el proceso se determinaron cuatro familias; Nymphalidae (80%), Hesperiidae (6.7%), Pieridae (6.7%) y Riodinidae (6.7%). Finalmente, se concluye que la familia Nymphalidae es la más prevalente representando una amplia fuente para el desarrollo de futuras investigaciones.

The quinoa pest Copitarsia incommoda (Walker, Lepidoptera: Noctuidae) is a cause of significant damage, and it is thus critical for Andean countries to have access to phenological models to maintain production and food safety. These models are key components in pest control strategies in the context of global warming and in the development of sustainable production integrating agroecological concepts. Phenological models are mainly based on outlining the relationship between temperature and development rate. In this study, we investigated the combined effect of protein content within the diet (artificial diet; artificial diet with −20% protein; artificial diet with +20% protein; natural quinoa diet) and temperature (12, 16.9, 19.5, 22.7, 24.6°C) as drivers of the development rate. Our study supports the literature, since temperature was found to be the main driver of the development rate. It highlights the significant role played by protein content and its interaction with temperature (significant effects of temperature, diet, and diet:temperature on development time using GLMs for all foraging life stages). We discuss the implications of such drivers of the development rate for implementing and applying phenological models that may benefit from including factors other than temperature. While performance curves such as development rate curves obtained from laboratory experiments are still a useful basis for phenological development, we also discuss the need to take into account the heterogeneity of the insect response to environmental factors. This is critical if pest control practices are to be deployed at the optimal time.
