Lepidic predominant adenocarcinoma and invasive mucinous adenocarcinoma of the lung exhibit specific mucin expression in relation with oncogenic drivers

Abstract: LPA and IMA exhibit specific mucin expression profiles, with MUC1 being associated with LPA, while MUC5B, MUC5AC, and MUC6 were associated with IMA. Hnf4α expression and EGFR and KRAS mutations may play a role in mucin expression profiles of these lung adenocarcinoma subtypes.

“…As previously reported, non‐TRU‐type adenocarcinomas, such as IMA, mostly expressed MUC5AC and MUC5B, whereas TRU‐type ones did not . However, in our study, most of the EGFR ‐mutated mucinous adenocarcinomas expressed only MUC5B.…”

“…The expression patterns of MUC1 and MUC5AC were different between EGFR ‐mutated and ALK ‐fused adenocarcinomas . Furthermore, MUC1, MUC5AC, MUC5B and MUC6 expressions differed between IMA and lepidic adenocarcinoma as well as according to the TTF‐1/HNF‐4a status . However, the mucin profiles of EGFR ‐mutated mucinous adenocarcinoma remain unclear.…”

“…Given that the differentiation of lung adenocarcinomas with mucinous features (pulmonary mucinous adenocarcinomas) from metastatic lesions is often challenging on a morphologic basis alone, multiple groups have studied the role of IHC in this context. In particular, distinguishing a metastasis from a pancreatic primary from invasive mucinous adenocarcinoma of the lung is far more challenging, given the similar immunoprofiles (focal caudal type homeobox 2 (CDX2) and cytokeratin 20 [ Table 8]) [140][141][142][143][144][145][146][147][148][149][150][151][152][153][154][155][156][157][158][159]161 ; furthermore, a lepidic growth pattern, which is characteristic of invasive mucinous adenocarcinoma (Fig. 12), is also often identified in pancreatic ductal adenocarcinoma metastatic to the lung (Fig.…”

“…Recently, it has been demonstrated that MUC5B serves as a marker for the non-terminal respiratory unit (TRU) subtype and the invasive mucinous adenocarcinomas associated with KRAS mutations [ 60 , 61 ]. As mentioned above, MUC5B polymorphism is associated with pulmonary fibrosis [ 21 , 22 , 23 ], and presumably, mucin 5B is overexpressed in IPF lung tissue.…”
