To ascertain and clarify the effectiveness and advantages of the geometric staged excision technique for the removal of lentigo maligna (LM) and lentigo maligna melanoma (LMM).Design: This was a retrospective review of a patient database composed of 293 cases of LM and LMM.Setting: The Laser and Dermatologic Surgery Center in St Louis, Missouri, an academic-affiliated, private dermatologic surgery center.Patients: All patients with a diagnosis of LM and LMM treated by staged excision from 1999 to 2007. Main Outcome Measures:The overall rate of recrudescence, margins required for clearance, stages re-quired for clearance, and lesional characteristics were examined.Results: The rate of recrudescence after geometric staged excision was 1.7% (4/239), with a mean of 32.3 months of follow-up. The mean margin to clearance after excision was 6.6 mm for LM and 8.2 mm for LMM. A total of 11.7% of LMM was initially diagnosed as LM on biopsy, with the invasive component discovered during the excision.Conclusions: Geometric staged excision is an optimal method of removal of LM and LMM given its low rate of recrudescence and ability for complete examination of the peripheral and deep margins of the specimens.

ÖzetLentigo malina, melanoma in-situnun alt tipidir. Genellikle 50-80 yaş arasında görülür ve deri kanserlerinin %10 ile 15' ini oluşturur. LM, atipik, pigmente, maküler lezyonların geleneksel tanımıdır.LM aşırı güneş hasarlanmasıyla oluşur. Genellikle yaşlı hastalarda ve yüz bölgelerinde görülür. Atipik, pigmente, maküler lezyonlarda histolojik tanı , solar lentigodan in-situ melanoma (lentigo maligna pattern) veya invazive melanomaya (lentigo maligna melanoma) kadar geniş aralığa sahiptir. İnsizyonel biyopsilerde solar lentigo, pigmente aktinik keratoz veya retiküle seboreik keratozun mevcudiyeti klinisyeni ve patoloğu yanıltabilir. Onlar lentigo malina bulunmadığını düşünebilirler. Bu yüzden bu vakalarda eksizyonel biyopsi gereklidir. LM is a traditional term for atypical pigmented macular lesions. LM occurs on severely sun damaged skin and it is usually on the face of elderly patients. Histological diagnosis of atypical pigmented macular lesions have extensive range from solar lentigo to in-situ melanoma (lentigo maligna pattern) or invasive invasive melanoma (lentigo maligna melanoma). The presence of solar lentigo, pigmented actinic keratosis, or reticulated seborrheic keratosis in incisional specimen could misguide the pathologist and clinician. they might think that lentigo maligna is not present. Therefore, excisional biopsy is needed in such cases. Our aim in presenting this case was to emphasize the importance of clinicopathologic correlation and to attract attention on the lesions adjacent to lentigo maligna which can lead to a misdiagnosis. Introduction: Lentigo maligna (LM) is a rare form of in situ melanoma, frequently seen as a large patch in elderly patients. The aim of this study was to assess clinical and dermoscopic features of LM. Material and Methods: A retrospective study of LM patients presenting to our center between July 2007 and July 2017 was performed. Demographic data, anatomical location, laterality, diameter, Clark level, Breslow stage, “ABCD” signs and dermoscopic features were registered. Facial versus extrafacial LM were compared. Results: We found 21 LM, of which 12 had an extrafacial location and 9 a facial location. Half of the extrafacial lesions were located on an upper limb. The median age at diagnosis was 63 years (ranging from 38 to 84 years). Most LM cases were female (16/21) with phototype II (13/21). More than half of the patients (11/21) had a history of a skin neoplasm or actinic keratosis. The median diameter found was 6 mm (interquartile range = 4.5 mm), ranging from 1 to 15 mm. Five lesions were invasive (median Breslow depth of 0.2 mm), and 4 of them were extrafacial. Discussion: In this study LM was more frequently found in an extrafacial location and as a small patch with a 6-mm diameter medium. The epidemiology of LM/LM melanoma might be changing. Full body examination and dermoscopy are of the utmost importance for the diagnosis. Dermatologists should be aware and search for small lesions outside the face and neck, particularly in middle-aged female patients with photo-damaged skin.
