Objetivo: Analisar memórias sobre a alimentação de pessoas idosas que foram hospitalizadas em uma Unidade de Terapia Intensiva. Material e Método: Estudo qualitativo, realizado em um hospital público de Salvador, Bahia, Brasil, com quatorze pessoas idosas que foram hospitalizadas em unidades de terapia intensiva entre janeiro de 2013 e maio de 2015. Adotaram-se os seguintes critérios: ser idoso, ter permanecido na UTI por mais de 72 horas, ter se alimentado por via oral e com condições cognitivas para participar de acordo com o Mini Exame do Estado Mental (MEEM). Assim, para os idosos sem escolaridade foram considerados aptos aqueles que apresentavam uma pontuação superior a 18, e 26 para aqueles com mais de oito anos de estudo. Os dados foram coletados por meio de entrevista semiestruturada e analisados através do software estatístico Alceste. Em todos os casos, foram fornecidas informações sobre o protocolo da pesquisa e solicitada a assinatura do Termo de Consentimento Livre e Esclarecido. Resultados: Identi cou-se a classe lexical Memórias de idosos sobre a alimentação, que revela os hábitos alimentares dos idosos, os alimentos preferidos, o ritual de preparo desses alimentos; as proibições sofridas pelo adoecimento crônico e a alimentação no hospital. Conclusão: A alimentação tem importante signi cado na vida destas pessoas idosas. Comer representa um prazer relacionado com suas memórias e identidades culturais.

Home-Dwelling Older Adults’ Day-to-Day Community Interactions: A Qualitative Study

Background: Many home-dwelling older adults present abnormal behaviours related to dementia or to non-dementia cognitive impairment (e.g., agitation, anxiety, apathy, etc.). Because many older adults live at home alone or are able to hide any signs of abnormal behaviours from others, the non-healthcare workers who interact with older adults on a daily basis are key actors in detecting those behaviours and orienting older adults towards appropriate support services. To the best of our knowledge, no studies to date have explored the daily interactions experienced between older adults and the various non-healthcare workers whom they regularly encounter in the community. This work aimed to identify the non-healthcare workers who are regularly in direct contact with older adults during their day-to-day activities and then develop specific training for these workers on the subject of abnormal behaviours among the elderly. Methods: This qualitative and ethnographic study asked 21 home-dwelling older adults aged 65 years old or more to answer open-ended questions. Sixteen had no self-reported cognitive impairments, and five had a probable or diagnosed slight cognitive impairment or mild-to-moderate dementia. A thematic analysis of the data was carried out. Results: The non-healthcare workers who spent the most time with older adults with and without reported cognitive impairments were those working in cafés or tea rooms and leisure or activity centres. Conclusions: In view of the significant amounts of contact between home-dwelling older adults and non-healthcare workers, it seems necessary and sensible to increase non-healthcare workers’ knowledge about abnormal behaviours, especially by offering them training. The proactive detection and identification of older adults’ abnormal behaviours by non-healthcare workers will ensure earlier care and reduce avoidable hospitalisations, institutionalisations and costs linked to geriatric healthcare.

Anemia Management in Rural Haitian Children: A Mixed Methods Study

Introduction We examined factors influencing anemia outcomes in rural children following implementation of a prevention program. Method Mixed methods study of children, parents, and clinicians utilized statistical modeling and content/ethnographic analysis. Retrospective chart abstraction evaluated treatments administered and measured hemoglobin in children aged 6 to 59 months ( n = 161). Prospective interviews/questionnaires examined parent ( n = 51) and clinician ( n = 19) perceptions. Results Anemia prevalence decreased by 21.2%. Predictors of increased hemoglobin were clinic visit number and age at first visit. Once anemia improved, children were likely to remain improved ( P = .65). Despite favorable program perceptions, stakeholders emphasized ecological barriers, including social disadvantage and local practices. Discussion Socioeconomic factors prevented guideline concordant behaviors. Persistent attention to intrapersonal, interpersonal, and community social determinants is a sine qua non for successfully managing the epidemic. The first step to provide culturally congruent care is to explicitly acknowledge that guideline-concordant behaviors are often complex.
