Adamantinoma is a rare low-grade malignant bone tumor of epithelial origin. Metastatic adamantinoma has been reported to be resistant to chemotherapy. We report a case of metastatic adamantinoma to the lung, 10 years after the initial diagnosis of tibial mass. The patient received radiation therapy to the lung with partial response. A surveillance PET scan revealed progression of the lung mass and biopsy confirmed to be progressive residual metastatic adamantinoma. He received carboplatin and etoposide for 7 months and achieved a partial response. Four months later, PET scan showed disease progression. We started him on sunitinib, a multikinase inhibitor. He achieved a good partial response for 3 years. He died due to pneumonia at the age of 72.

Adamantinoma is an uncommon tumour of low grade malignancy in terms of biologic aggressiveness; metastasis occurring many years after amputation. A young male underwent an above knee amputation of left leg for adamantinoma of tibia. He returned with pain and swelling in his right thigh five years later. Investigations revealed an adamantinoma in the diaphysis of the right femur. The unusual presentation of this adamantinoma at an uncommon site threw up interesting insights into the nature of this lesion. In particular, the metachronous origin of the adamantinoma in the femur is discussed in this report.

Presentamos el caso de una paciente de veinte años de edad quien comienza con fractura patológica de la tibia derecha por un trauma de baja energía, con estudios radiológicos e histopatológicos que confirmaron la presencia de un adamantinoma. Fue manejado quirúrgicamente mediante resección amplia del segmento tumoral y reconstrucción con aloinjerto intercalar de tibia fijado con un clavo endomedular de estabilidad multidireccional sustentado con una placa antirrotatoria proximal. Los resultados radiológicos y clínicos fueron satisfactorios. Adamantinoma of tibia: Surgical management with intercalary allograft.
