Left Ventricular Hypertrophy in Men with Normal Blood Pressure: Relation to Exaggerated Blood Pressure Response to Exercise

Abstract: Even in the absence of hypertension, exaggerated blood pressure responses during exercise testing suggest a probability of 0.64 (95% CI, 0.41 to 0.83) of LVH, a finding associated with the cardiac "end-organ" manifestations of hypertension.

“…Some papers showed that normotensive subjects with exaggerated blood pressure response to physical effort were highly likely to develop sustained arterial hypertension later on, 5,6 as well as high prevalence of LVH. 7 There are divergences regarding neurohumoral influence over LVH in this specific group of subjects, with some data in the literature pointing out to sympathetic hyperactivity as a triggering factor. 8 Nevertheless, there seems to be an independent and significant correlation between the aldosteron/renin ratio and an increase in systolic blood pressure (SBP) during exercise.…”

“…Moreover, a higher proportion of MH individuals displayed a peak BP level considered to be a risk marker for cardiovascular disease. 5 Type II DM patients with MH have increased albuminuria and left ventricular mass, even with office BP levels in the normotension range. 1 One possible explanation for this observation is that, during ordinary daily activities, these patients have an exaggerated further increase in the already elevated BP levels during the course of the day.…”

“…1 Patients performed exercise treadmill test (standard Bruce protocol) utilizing a computerized database. 5 Heart rate, BP and estimated workload (METs) were recorded midway through each stage of the exercise protocol, at peak exercise, and at 1, 2 and 4 minutes after cessation of exercise. The BP increment was calculated as the difference between BP at rest and at peak exercise.…”

“…A história familiar de HAS é fator predisponente para elevação da pressão arterial (PA) e para alterações estruturais e hemodinâmicas, que podem preceder o diagnóstico de HAS [5][6][7][8][9] . O enfoque destes indivíduos deve ser diferente para prevenir o aparecimento de HAS e, talvez, modificar a sua história natural.…”
