The systolic and diastolic left ventricular (LV) function was studied by M-mode echocardiography in 60 patients with β-thalassemia (mean age ± SD, 17.1 ± 7.5 years) and 30 healthy controls (15.4 ± 3.8 years). In thalassemic patients, echocardiograms were obtained 48 h posttransfusion, with a mean hemoglobin level of 12.4 ± 0.9 g/dl. To examination time, thalassemic patients had received 30-774 blood units (318 ± 176). Congestive heart failure (CHF) was present in 14 thalassemic patients (19.6 ± 3.4 years), while 46 (16.3 ± 8.2 years) had no clinical signs of CHF. Global LV function study showed enlarged LV dimensions in thalassemic patients with CHF (p < 0.001) and similar cavity size in controls and patients without CHF (p = NS). The same was true for velocity measurements, while diastolic LV indices had similar values in all groups (p = NS). Segmental LV function study showed no significant differences in systolic and diastolic LV posterior wall behavior between thalassemic patients and controls, and even more, between thalassemic patients with and without CHF, while it was independent of iron load. These findings indicate that global and segmental LV function in thalassemic patients remain within normal limits until the final stages of the disease. CHF onset marks the deterioration of LV systolic performance, while global and segmental diastolic indices do not change significantly. The above findings question the role of iron overload in the development of CHF in β-thalassemia.

Summary: Left ventricular (LV) performance was studied in young patients with severe chronic anemia due to 6-thalassemia major, intermedia, and in healthy control subjects. M-mode echocardiogms were recorded in each patient and semiautomatic computerized analysis of the tracings provided data relating to LV performance. Then a statistical analysis of the difference between each specific thalassemic group and the normal subjects was made using Student's r-test for unpaired data. The study showed that cardiac dysfunction is more serious in major than in intermediate 6 thalassemia. A follow-up one year later showed a progressive deterioration of the cardiac indices, in spite of treatment with desfemoxamine. A handgrip test was performed in the follow-up study, which permitted us to distinguish different groups relative to the changes in LV performance indices. Our findings indicate that echocardiography provides a simple noninvasive means for assessing changes in the cardiac structure and function, which should also prove useful in the serial evaluation of patients at risk of developing myocardial iron deposition.

SummaryBuckground and hypothesis: An important complication of beta-thalassemia is iron deposition in cardiac tissues resulting in fibrosis and dysfunction. Our aim was the investigation of the possible clinical effect of iron loading in the heart of patients with beta-thalassemia prior to the appearance of symptoms of depressed systolic function.Methods: Thirty-five patients with beta-thalassemia, of whom 24 had the major type (Group 1) and 1 l had the intermedia type (Group 2) were studied. Eleven age-and gendermatched controls were also studied (Group 3). All patients were evaluated echocardiographically and were shown to have normal left ventricular systolic function and dimensions. Serum fenitin, atrial natriuretic peptide (ANP), left atrial diameter (LAD), peak early mitral inflow velocity (E), peak late mitral inflow velocity (A), EJA ratio, deceleration time of the mitral inflow E wave (DT), and isovolumic relaxation time ( IVRT) were measured.Results: Univariate analysis showed that both groups of patients had similarly increased LAD and ANP plasma levels. Group 1 had a higher E/A ratio (2.27 & 0.88) SS than Group 2 (1.69 2 0.47, p = 0.05) and Group 3 (1.50 * 0.38, p = 0.01).
