Left ventricular aneurysm repair: an assessment of surgical treatment modalities

Abstract: Left ventricular aneurysm repair is an important therapeutic intervention and can be performed with reliable results, regardless of repair method, either linear or circular. Long term results revealed better functional status in circular repair group. Concomitant coronary revascularization reduced the incidence of low cardiac output state. Performance of the unaffected regions of myocardium was found to be an important determinant of both early and late outcome.

“…On purely theoretic considerations [9][10][11], patch remodeling of postinfarction LV aneurysms improves ventricular geometry, thus contributing to better early and late outcome [12][13][14]. But some retrospective clinical studies failed to demonstrate any difference between linear and patch repairs [15][16][17][18][19][20].…”

“…Etiologies that might have a role in early mortality are the absence of angina, presentation with dyspnea, long aortic cross clamping time, recent myocardial infarction, lower ejection fraction, multivessel disease, right coronary artery disease, and incomplete revascularization in earlier studies. 9,10) In our study, it seemed additional revascularization was beneficial, and the presence of thrombus had negative effects on early mortality in univariate analysis. Nevertheless, neither had an effect in multivariate analysis.…”

“…Nevertheless, the effects of these techniques on mortality are controversial. 8,9,16) In a study which compared classic linear repair vs. endoventricular patch plasty, the early mortality rates were found to be 7.8% vs 20%, respectively. 17) In our study, classic aneurysmectomy and plication were used as the technique for LVA surgery in a vast majority of patients.…”
