Left atrial myxoma associated with acute myocardial infarction and multiple cerebral infarctions: Report of a case

Abstract: We report a case of left atrial myxoma associated with acute myocardial infarction (AMI) in a 63-year-old man. Percutaneous coronary intervention was performed immediately, and we removed white intracoronary particles by using a thrombectomy catheter. The postinterventional course was uneventful; however, on hospital day 5, pathological examination revealed platelet thrombus and myxomatous tissue from the particles aspirated by thrombectomy catheter, and transesophageal echocardiography showed a gelatinous mas… Show more

“…Nineteen (14.3%) patients had other than cerebral artery embolisms, with systemic multiple emboli in 4 (21.1%) [ 55 , 63 , 71 , 80 ] , peripheral multiple emboli in 3 (15.8%) [ 19 , 72 , 85 ] and solitary arterial embolus in 12 (63.2%) patients [ 9 , 11 , 13 , 16 , 21 , 35 , 41 , 43 , 60 , 76 , 84 ] . “…It was the only diagnostic tool in 79 (87.8%) patients and as an adjunctive to other imaging in 11 (12.2%) patients ( Table 9 ). In one patient, the pathology of the aspirated materials by catheterized thrombectomy on day 5 after admission revealed platelet thrombus and myxomatous tissue, which aroused the suspicion of a cardiac myxoma origin [ 41 ] . Both transthoracic and transesophageal echocardiography showed a respective false negative result in one patient each [ 10 , 19 ] , the cause of the latter was a complete detachment of the cardiac myxoma [ 19 ] .…”

“…[14] The sensitivity of transthoracic echocardiography in diagnosing myxoma is 95%, and the sensitivity in transesophageal echocardiography is 100%. [15] However, when the patient is critical, the poor quality of the images caused by position-restricted bedside instrument examinations or the atypical ultrasound images of myxoma can still lead to misdiagnosis of myxoma, [16] which delays treatment and leads to a life-threatening situation. [17]…”

“…They are rarely found at other sites within the cardiac cavities, such as the ventricles, although they occasionally involve a cardiac valve [1]. Their clinical presentations can be varied, with local cardiac hemodynamic consequences, valvular insufficiency or even embolic phenomena [2]. We report a rare case of left atrial and right ventricular and left ventricular myxomas.…”
