BackgroundThere are no published studies on the simultaneous effect of extent and location of positive surgical margins (PSMs) on biochemical recurrence (BCR) after robot-assisted laparoscopic prostatectomy (RALP). The aim was to report the incidence, extent, and location of PSMs over the inclusion period as well as the rates of BCR and cancer-related mortality, and determine if BCR is associated with PSM extent and/or location.MethodsRetrospective review of 530 consecutive patients who underwent RALP between 2003 and 2012. Kaplan-Meier (KM) survival analyses and Cox regressions were performed to determine variables associated with BCR.ResultsFor the 530 operated patients, evaluated at a median of 92 months (IQR, 87–99), PSMs were observed in 156 (29%), of which 24% were focal. Out of 172 PSMs, 126 (73%) were focal and 46 (27%) were extensive. The KM survival using BCR as endpoint was 0.81 (CI, 0.78–0.85) at 5 years and was 0.67 (CI, 0.61–0.72) at 10 years; and using cancer-related mortality as endpoint was 0.99 (CI, 0.99–1.00) at 5 years and 0.95 (CI, 0.92–0.98) at 10 years. Multi-variable analysis revealed the strongest predictors of BCR to be Gleason score ≥ 8 (HR = 7.97; CI, 4.38–14.51) and 4 + 3 (HR = 3.88; CI, 2.12–7.07), lymph nodes invasion (HR = 3.42; CI, 1.70–6.91), pT stage 3b or 4 (HR = 3.07; CI, 1.93–4.90), and extensive apical PSMs (HR = 2.62; CI, 1.40–4.90) but not focal apical PSMs (HR = 0.86; CI, 0.49–1.50; p = 0.586).ConclusionExtensive apical PSMs significantly increased the risk of BCR, independently from pT stage, Gleason score and lymph nodes invasion, while focal apical PSMs had no significant effect on BCR.Electronic supplementary materialThe online version of this article (10.1186/s12885-018-5229-1) contains supplementary material, which is available to authorized users.

Мета дослідження: вивчення та порівняльний аналіз результатів радикального хірургічного лікування у хворих на локалізований рак передміхурової залози (РПЗ), які перенесли ендоскопічну радикальну простатектомію (ЕРПЕ) на умовних етапах впровадження та становлення цих методик у клініці. Матеріали та методи. Здійснено оцінювання результатів лікування локалізованого РПЗ у 261 пацієнта, які перенесли ЕРПЕ у клініці ДУ «Інститут урології НАмН України» протягом 2012-2016 рр. включно. Хворі були розподілені на дві групи за деперсоніфікованим календарним принципом. До групи 1 увійшли 99 (37,93%) пацієнтів, яким ЕРПЕ виконували у період з 2012 до 2014 р. (умовний етап впровадження ЕРПЕ у клініці). До групи 2-162 (62,07%) пацієнта, які перенесли ЕРПЕ в період з 2015 до 2016 р. (умовний етап становлення ЕРПЕ у клініці). Проаналізовано клінічні, лабораторні, розрахункові та патоморфологічні відомості по кожному хворому. Дані оброблені методами дескриптивної, параметричної та непараметричної статистики (t-тест Стьюдента, U-критерій манна-Уїтні, χ² Пірсона) та ін. Результати. Пацієнти у групах 1 та 2 статистично значуще не відрізнялись за віком (p=0,342), масою тіла (p=0,839), значенням загального простат-специфічного антигену перед проведенням хірургічного лікування (p=0,464), об'ємом передміхурової залози (ПЗ) (p=0,507), наявністю середньої долі ПЗ більше 1 см (p=0,828) або післяопераційно (п/о) ліжко-добою (p=0,613). У групі 1 виявлено статистично значуще збільшення кількості хворих, яким до виконання ЕРПЕ проводили неоад'ювантну терапію по відношенню до групи 2 (p=0,012). Частота проведення гемотрансфузій становила 17,17% vs 6,79%, об'єм інтраопераційної крововтрати-500 [235; 800] мл vs 250 [150; 400] мл, медіана часу катетеризації сечового міхура (См)-12 [9; 15] vs 11 [8; 14] діб і ΔHb становила-24 [-33;-14] г/л vs-19 [-28,25;-11] г/л у групі 1 проти групи 2 відповідно. Було виявлено 69 ускладнень у групі 1 у 38 (38,38%) пацієнтів і 75 ускладнень у групі 2 у 56 (34,57%) хворих. в обох когортах пацієнтів переважали генітоуринарні ускладнення, поширеність яких у групі 1 становила 34,34%, а в групі 2-29,01%. З усієї сукупності п/о ускладнень найвищу частоту виникнення мала п/о негерметичність везико уретрального анастамозу (вУА): 18,18% для групи 1 і 10,49% для групи 2. в обох групах переважали «малі» ускладнення (Clavien I-II) 58,59% та 35,8%, а «великі» ускладнення (Clavien III-IV) були зафіксовані в 11,11% та 10,49% у групах 1 та 2 відповідно. Кількість хворих, у яких виникли п/о ускладнення у досліджуваних групах була зіставною (p=0,534), однак їхній розподіл відрізнявся. Заключення. Надбання досвіду у виконанні ЕРПЕ співробітниками інституту привело до статистично значущого зменшення часу післяопераційної катетеризації См (p=0,018), об'єму інтраопераційної крововтрати (p<0,001), частоти проведення гемотрансфузій (p=0,009), зниження загальної поширеності виникнення післяопераційних ускладнень (p<0,001), а також поширеності «малих» (Clavien I-II) післяопераційних ускладнень (p<0,001) без статистично значущого впливу на ана...

Purpose: We compared retropubic radical prostatectomy (RRP) with various laparoscopic radical prostatectomy procedures with RRP as previous performed in our institution. Materials and Methods: Demographics, perioperative and functional outcomes of 78 patients that underwent modified RRP (mRP; N=53) or established RRP (eRP; N=25) at our institution from January 2013 to December 2018 were evaluated retrospectively. Postoperative incontinence and erectile dysfunction are involved functional outcomes. During the modified procedure, with preserving endopelvic fascia, the prostate was dissected in an antegrade fashion with bilateral nerve sparing, and then urethrovesical anastomosis was performed with continuous suture. Results: The mean age was older in mRP (68.7±5.3 years) than mRP (65.9±5.0 years) and mean prostate volume was larger in mRP (40.1±18.6 mL) than eRP (30.4±14.0 mL). Mean operative time was longer in eRP (227.0±111.1 minutes) than mRP (154.6±31.6 minutes) and estimated blood loss and complicate rate were similar in 2 groups. The pathologic stage was all T2 stage in eRP, however, in mRP T2 stage was 31 and T3 stage was 22. The pathologic Gleason score was higher in mRP than eRP (p=0.001). Positive surgical margin was significantly higher in mRP, however, biochemical recurrence was insignificantly higher in mRP. Incontinence rates at 3 and 12 months after eRP decreased from 96.0% to 28.0% and after mRP decreased from 49.1% to 7.5%. Overall postoperative potency rate at 12 months was significant different in eRP and mRP groups (8.0% and 34.0%). Conclusions: The mRP was found to have favorable functional outcome and short operative time. This technique might be adopted by inexperienced urologic surgeons as a standard procedure.
