S Bromfenac Corneal perforation: case reportA 66-year-old man developed corneal perforation following administration of bromfenac.The man who had undergone cataract surgery of left eye 2 weeks ago, presented with redness and pain in eye for one week. He had been taking bromfenac eye drops 0.09%[dosage and duration of treatment to reaction onset not stated; indication not clearly stated] post surgery. Opthalmoscopic and biomicroscopic examination and optical coherence tomography revealed severe corneal melting of the vertex with desmetocele.The man was hospitalised with the installation of a bandage contact lens and was started on retinol, hyaluronic acid drops and piperacillin and levofloxacin. Spontaneous seidel sign with aggravation of lesion was observed on 6 th day, and on 10 th day hernia of the iris with hypothalamy was observed. He underwent urgent surgical treatment for reduction of the hernia by injection of viscous material, followed by reconstruction of an anterior chamber. Follow up examination revealed a leak proof but remodelled cornea and reformed anterior chamber. The man was continued on topical corticosteroid treatment.Author comment: "Corneal perforation under NSAIDs is rare but serious. It puts the functional prognosis at risk. This case observed by us, together with the reported cases involving diclofenac sodium, ketorolac and also indomethacin suggest that careful prior observation of the condition of the cornea is necessary before the use of NSAID eye drops."
