Laughter‐Induced Left Bundle Branch Block

Abstract: We present the case of a patient with ischemic heart disease and intermittent left bundle branch block, reproducibly induced by laughter. Following treatment of ischemia with successful deployment of a drug-eluting stent, no further episodes of inducible LBBB were seen. Transient ischemia, exacerbated by elevated intrathoracic pressure during laughter, may have contributed to onset of this phenomenon.

“…It has been reported with exercise, tachycardia, ischemic and hypertensive heart disease, hyperkalemia, drugs, anesthesia, pulmonary embolism, chest trauma, and cardiac interventional procedures [ 8 - 12 ]. Apart from transient LBBB, RBBB, bifascicular block, and advanced AV block have also been reported [ 13 - 15 ].…”

“…He subsequently had triple vessel disease on coronary angiogram. Successful percutaneous intervention to the left circumflex artery was done after which the LBBB was not induced by laughing, raising the likelihood that the mechanism of LBBB was ischemia-driven [10]. To the best of our knowledge, there is no prior documented case of control of LBBB with coughing.…”
