Late type III endoleak from fabric tears of a zenith stent graft: report of a case

Abstract: We report a case of late type III B endoleak from a fabric tear in the main body of a Zenith bifurcated stent-graft approximately 19 months after implantation. A follow-up computed tomography (CT) scan showed gradual growth in the size of the aneurysm, but no apparent endoleak was recognized. However, the CT scan had demonstrated a mosaic pattern in the aneurysm sac, which could indicate an endoleak from a fabric tear. The defect was repaired by implantation of body extensions, but the patient died 5 h after t… Show more

“…We did not know the reason for the fabric defect after 14 months after the deployment. In the literature, there are reports about type 3b endoleaks with other aortic stent graft devices [911] but there was only one case report about early type 3b endoleak associated with Endurant endograft [12]. Our case is the first case of a late type 3b endoleak reported with an Endurant stent graft.…”

“…Our experiences regarding type III endoleak, including graft fabric tears, are published elsewhere. 18 Five type Ia endoleaks were successfully treated via endovascular procedures, and 1 patient was treated via replacement of the endograft with a bifurcated graft in an open procedure. Type II endoleak is reportedly more common, the incidence of which varies between 10% and 44%, which is consistent with the present results.…”

“…Type IIIb endoleaks are relatively rare and reports are scarce. Though these reports on type IIIb endoleaks in stent grafts involve multiple currently available devices [ [6] , [7] , [8] ], many of them are reported when using the Zenith [ 7 , 9 , 10 ].…”
