Background: Penile injection of foreign materials is an obsolete practice often performed by non-medical personnel in order to enlarge penile size. Methods: A systematic review of the literature from 1956 to 2022 was conducted in accordance with the general guidelines recommended by the Primary Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) statement. We included full papers published from 1956 to 2022. We also described a case report of a 23 year old Bulgarian male affected by penile paraffinoma who underwent a 2-stages surgical technique. Results: A total of 152 cases have been reported, with a median age of 37.9 ranging from 18 to 64 years. Six different techniques have been described in the whole literature: bilateral scrotal flap, simple excision of the paraffinoma with primary closure, two-stage scrotum skin flap, medial prepuce-soprapubic advancement flap technique and penile reconstruction using split thickness skin graft (STSG) or full thickness skin graft (FTSG). An analysis of the distribution among early and late complications was then carried out. Conclusion: In our experience, among the variety of surgical techniques described, a two-stage penile reconstruction using scrotal skin results in excellent cosmetic and functional outcomes, with a low rate of complications.

A New Modified Bipedicle Scrotal Skin Flap Technique for the Reconstruction of Penile Skin in Patients with Paraffin-Induced Sclerosing Lipogranuloma of the Penis

PurposeWe introduce a new modified penile skin reconstruction technique to treat paraffin-induced sclerosing lipogranuloma of the penis. Materials and MethodsFrom 2017 to 2020, 49 patients underwent the procedure. Complete removal of the lipogranuloma-involved penile skin was performed. A subcutaneous tunnel was then created between a horizontal scrotal incision and a proximal penile circumferential incision. The denuded penis was pulled through the tunnel, and a subcoronal and longitudinal dorsal penile suture line was made. An inverted V-shaped incision was made on the scrotum on the ventral side of the penis, followed by longitudinal closure. Outcomes and complications of the procedure were retrospectively studied. The long-term effect of surgery on sexual function and overall satisfaction was measured using a patient-reported questionnaire, which was completed by 30 patients. ResultsThe overall complication rate was 26.5%. Clavien-Dindo grade 1, 2, 3a, 3b, 4, and 5 complications occurred in the postoperative period 5, 0, 8, 1, 0, and 0 times, respectively, in 13 patients. Surgery was successful in 27 (90%) patients, according to the patient-reported questionnaire. Erectile dysfunction, pain/tension during erection, premature ejaculation, and penile lymphoedema were observed in 2, 3, 1, and 1 patients, respectively. All patients reported sexual intercourse ability. ConclusionsThe type of penile skin reconstruction after the removal of sclerosing lipogranuloma of the penis is controversial. The reconstruction technique presented herein is an effective singlestage treatment option with a high success rate in patients with sclerosing lipogranuloma of the penis with intact scrotal skin.

Background: Paraffin injection into the penis is one way that people see it as increasing penis size. But many do not know that paraffin injection actually causes side effects in the form of penile paraffinoma. This study aimed to describe the management of paraffinoma using bilateral techniques. Case presentation: A 40-year-old married man came to the emergency room at Samuel J. Moeda Naval Hospital, Kupang, with complaints of pain and swelling of the penis for 6 hours. The pain felt to get worse when the penis was erected. The genital organs found swelling of the entire penile skin accompanied by tenderness and induration. The bilateral scrotal flap technique was because the scrotal skin had many blood vessels, good flexibility, little adipose tissue, thin skin, and could feel erogenous sensations almost the same as the physiological characteristics of the penile skin. Conclusion: Bilateral scrotal flap technique on the penile shaft is a surgical technique that is quite effective in eradicating paraffin from penile tissue.
