Late-onset Niemann–Pick disease type C overlapping with frontotemporal dementia syndromes: a case report

Abstract: The diagnosis of adult-onset Niemann–Pick disease type C (NPC) could be difficult because its primary symptoms [dementia and vertical supranuclear gaze palsy (VSGP)] are mainly seen in neurodegenerative dementias and progressive supranuclear palsy (PSP). Our patient with dementia and asymmetric parkinsonism resembled corticobasal syndrome and after the appearance of VSGP, the criteria of PSP were fulfilled too. Cerebellar symptoms appeared late during the course of the disease, leading to the diagnosis of NPC … Show more

“…To the best of our knowledge, patient 2 is only the third case to be reported with age of onset at or after 55 years old 14,15 and only the second case to be reported with an age of diagnosis after 65 years old 16 . Both of our patients had a delay in diagnosis of 13 years, which is similar to prior reports in patients with NPC diagnosed >50 years of age, with a median delay in diagnosis of 13.25 years 3,14–19 …”

“…16 Both of our patients had a delay in diagnosis of 13 years, which is similar to prior reports in patients with NPC diagnosed >50 years of age, with a median delay in diagnosis of 13.25 years. 3,[14][15][16][17][18][19] For patients with a clinical profile suggesting NPC, oxysterols are the most widely used biomarkers with good accuracy. Plasma cholestane-3β,5α,6β-triol, 7-ketocholesterol are elevated, with cholestane-3β,5α,6β-triol being the preferred oxysterol biomarker because of its high specificity and sensitivity for NPC.…”

“…Like Type A, Type C is most frequently characterized as a pediatric disorder, but there are 2–3 times more cases of adolescent and adult onset ( Yanjanin et al, 2010 ; Wassif et al, 2016 ). However, Type C is primarily a neurodegenerative disorder, and its diagnosis is often confounded by neurological signs common to a variety of diseases ( Balázs et al, 2019 ; Rego et al, 2019 ; Johnen et al, 2018 ). The signs include vertical gaze palsy, cerebellar ataxia, and dystonia, as well as mild cognitive deficits in visual memory, verbal fluency, and executive function.…”
